摘要
目的研究初发2型糖尿病患者、正常人群25-(OH)-D3与胰岛素抵抗、胰岛β细胞分泌功能的关系,初步探讨2型糖尿病的发病机制。方法收集初发2型糖尿病患者77例和正常对照46例,所有研究对象均测其25-(OH)-D3、血糖、血脂、糖化血红蛋白(HbA1c)及胰岛素水平。用稳态模式评估胰岛素抵抗指数(HOMA-IR)和胰岛β细胞分泌功能(HOMA-B)。结果初发2型糖尿病组的25-(OH)-D3、HOMA-B明显低于正常对照组,差异有高度统计学意义(P<0.001)。25-(OH)-D3与FBG、HbA1c呈负相关(P<0.01),与FIN、HOMA-B呈正相关(P<0.01),与HOMA-IR不相关(P>0.05)。结论在初发2型糖尿病患者中25-(OH)-D3水平显著降低,影响胰岛β细胞分泌功能,可能参与2型糖尿病的发病。
Objective To probe into the relationship between 25-(OH)-D3 and insulin resistance,islet β cell secretion function in newly diagnosed type 2 diabetes and normal group so as to study the pathogenesis of type-2 diabetes.Methods 77 newly diagnosed type-2 diabetic patients and 46 normal controls were collected and all subjects were measured for 25-(OH)-D3,blood glucose,lipids,glycosylated hemoglobin(HbA1c) and insulin levels.Homeostasis model was adopted to assess insulin resistance index(HOMA-IR) and islet β cell secretion function. Results The levels of 25-(OH)-D3 and HOMA-B of the newly diagnosed type-2 diabetics were significantly lower than those of the normal control group,and the differences were of high statistical significance(P<0.001).25-(OH)-D3 was negatively correlated with fasting blood glucose and HbA1c(P<0.01),positively correlated with fasting insulin and HOMA-B(P<0.01),not related to HOMA-IR(P>0.05). Conclusion 25-(OH)-D3 levels were significantly lowered in newly diagnosed type-2 diabetic patients which affects islet β cell secretion function and may be involved in the pathogenesis of type-2 diabetes.
