胃癌术后患者手术切口感染的危险因素分析 被引量:10

Risk factors for surgical incision infections in gastric cancer patients after surgery

摘要 目的探讨胃癌术后患者手术切口感染发生的相关危险因素,以期能为预防及降低术后切口感染的发生提供参考。方法回顾性分析412例胃癌手术患者临床资料,分析术后切口感染发生情况及相关危险因素,采用SPSS17.0软件进行统计分析,多因素分析采用logistic非条件回归分析。结果术后发生切口感染率9.47%,感染患者术后住院时间平均(21.28±5.83)d,未发生感染患者术后住院时间(15.92±5.22)d,差异有统计学意义(P<0.05);年龄、体质量指数、术前吸烟、低蛋白、TNM分期、手术方式、缝合切口人员工作年限、术中使用电刀、术前使用抗菌药物是与胃癌术后发生切口感染的危险因素;多因素logistic非条件回归分析显示年龄≥65岁、体质量指数≥24kg/m2、缝合切口人员工作年限<10年、术中使用电刀是引起患者术后切口感染发生的独立危险因素(P<0.05)。结论胃癌术后切口感染率较高,引起切口感染发生的危险因素较多,应针对性的进行干预以降低术后切口感染率。 OBJECTIVE To explore the related risk factors for postoperative surgical incision infections in the gastric cancer patients so as to provide guidance for the prevention of surgical incision infections.METHODS The clinical data of 412cases of patients with gastric cancer were retrospectively analyzed,then the related risk factors for the postoperative incision infections were analyzed,the statistical analysis was performed with the use of SPSS17.0 software,and the non-conditional multivariate logisticregression analysis was carried out.RESULTS The incidence of the postoperative incision infections was 9.47%;the average hospitalization duration of the patients with infections was(21.28±5.83)days,the patients without infections(15.92±5.22)days,the difference was significant(P0.05).The risk factors for the postoperative incision infections in the gastric cancer patients included the age, body mass index,preoperative smoking,hypoproteinemia,TNM staging,surgical procedure,working life of the suturing personnel,intraoperative use of electrotome,and preoperative use of antibiotics.Multivariate logistic regression analysis indicated that more than 65years of age,body mass index more than 24kg/m2,working life of the suturing staff less than 10years,and intraoperative use of electrome were the independent risk factors for the postoperative incision infections(P0.05).CONCLUSION The incidence of postoperative incision infections is high in the gastric cancer patients,and there are a variety of risk factors for the incision infections,it is necessary to take targeted interventions so as to reduce the incidence of postoperative incision infections.
