摘要
目的 :探讨人工全髋关节置换术治疗髋关节疾病的作用、手术方法、疗效及预后。方法 :应用美国Biomet公司人工全髋关节假体及手术器械 ,对 1 0 0例患者 (1 1 2髋 )施行全髋关节置换 ,其中非骨水泥型人工全髋关节 78髋 ,骨水泥型人工全髋关节 2 2髋 ,杂交固定型人工全髋关节 1 2髋 ,平均随访 5年。结果 :患者平均术后 3～ 4周离床活动 ,1例术后发生关节脱位 ,3例发生手术侧肢体深静脉栓塞。所有患者经 5年随访 ,假体无松动 ,无翻修 ,关节功能Harris评分均较术前优良。结论 :人工全髋关节置换术治疗类风湿性髋关节炎 ,髋关节骨关节炎 ,股骨头无菌性坏死 ,股骨颈骨折等疾病疗效显著 ,并发症少 ,很大程度地改善了病人关节功能 ,提高了术后生活质量。
Objective:To study the function,technique and prognosis of total hip replacement (THR) in the hip disorders.Methods:One hundred patients have been treated by THR with Biomet total hip system,including the implants with cement (22 hips),no-cement (78 hips),hybridize implants (12 hips).Results:All patients could move by working stick in 3～4 weeks after operation.Dislocation and the complication of DVT occurred in one case and three cases respectively after operation.All cases were fine by Harris-Score through 5-year follow-up.Conclusions:THR is particularly effective against osteoarthritis,rheumatoid-arthritis,avascular necrosis of femural head and femural neck fracture.
