摘要
目的分析膝骨关节炎关节损害影像学严重程度与关节滑膜水通道蛋白-4(AQP4)、P物质(SP)基因表达情况之间的关系,探讨骨关节炎疼痛的神经源性炎症机制。方法将36只雄性新西兰大白兔随机分为对照组和实验组,实验组进行骨关节炎造模,在大白兔膝关节腔注射高浓度福氏佐剂以制备骨关节炎模型。采用磁共振成像(MRI)扫描兔膝关节进行监测,12周后取关节滑膜组织,PCR检测SP和AQP4基因表达。结果骨关节炎大白兔随造模时间延长出现关节软骨变薄、滑膜增生等炎症及退行性改变;关节滑膜AQP4、SP基因表达随关节损害影像学严重程度逐渐升高。经统计学分析关节损害影像学严重程度和关节滑膜AQP4、SP基因表达之间存在相关性。结论 SP基因表达水平可能可以作为反映骨关节炎组织炎症和神经源性反应严重程度的指标;AQP4基因则可能可以作为反映骨关节炎组织水肿严重程度的指标,神经源性炎症可能在骨关节炎疼痛机制中发挥着重要作用。
Objective To analyze the relation between knee joint damage imaging severity and expressions of synovialaquaporin-4 (AQP4) and substance P (SP) gene and to explore neurogenic inflammatory mechanisms of osteoarthritis pain. Methods Thirty-six male New Zealand white rabbits were randomly divided intoexperimental group andcontrol group. Osteoarthritis modeling was carried out in the experiment group. A high concentration of Freund's adjuvant was injected into rabbit knee joint cavity to prepare osteoarthritis model. Rabbit knee jointswere monitored by MRI scanning for 12 weeks. After that, rabbit synovial tissues were collected for PCR in order to detect expression levels of SP and AQP4 gene. Results Results showed inflammatory and degenerative changes in rabbits with osteoarthritis, such as cartilage thinning and synovial hyperplasia, with the prolonged modeling. Ex- pression levels of synovial AQP4 and SP gene gradually increased with the imaging severity of joint damage. Sta- tistical analysis revealed a relation between the imaging severityof joint damage and the expression oi synovi- alAQP4 and SP gene. Conclusion Neurogenic inflammation may play an important role in pain mechanisms of osteoarthritis. SP gene expression levels may be used as an indicator to reflect the severity of osteoarthritis tissue in- flammation and neurogenic response. AQP4 gene may also be considered as an indicator to reflect the severity of osteoarthritis tissue edema.
