摘要 目的探讨基因芯片技术检测结核分枝杆菌耐药性的临床价值。方法回顾性分析685例结核病临床疑似病例的临床资料,采用晶芯结核分枝杆菌生物芯片技术进行检测。结果本组685例临床疑似病例中,阳性135例,阳性率为19.7%,基因芯片检测结果显示结核131例,非结核4例,阳性率为19.1%。耐药基因检测68例,利福平和异烟肼基因突变12例,总突变率为17.6%,其中11例为利福平和异烟肼均耐药,双重基因突变率为16.2%,另1例只耐利福平。利福平总突变率为17.6%,异烟肼总突变率为16.2%,耐多药率为16.2%。结论晶芯结核分枝杆菌生物芯片技术用于临床诊断结核病及其耐药性,能够缩短诊断时间,提高临床时效性,值得推广应用。 Objective To study the clinical value of the gene chip technology in detecting the drug resistance of My- cobacterium tuberculosis. Methods The clinical data of 685 cases with tuberculosis clinical suspected were retrospec tively analyzed and the jingxin biochip technology of Mycobacterium tuberculosis was performed for the detection. Results In the 685 clinical suspected cases,135 cases showed positive results,the positive rate was 19.7%,the detecting result of gene chip technology showed that 131 cases were tuberculosis and 4 cases were non-tuberculosis, and the positive rate was 19.1%, The resistance gene detection was performed in 68 cases,and there were 12 cases of gene mu- tations of rifampicin or isoniazid and the overall mutation rate was 17.6%,among them,11 cases were drug resistance of both rifampicin and isoniazid and the double mutation rate was 16.2% and 1 case was only drug resistance of ri- fampicin.The total mutation rate of rifampin was 17.6%,while the total mutation rate of isoniazid was 16.2%,and the multi-drug resistance rate was 16.2%. Conclusion The jingxin biochip technology of Mycobacterium tuberculosis used for clinical diagnosis of tuberculosis and detecting the drug resistance can shorten the diagnostic time and improve clin- ical timeliness,which is worthy of popularization and application.
