摘要
目的 探讨在无痛胃镜下使用圈套器将胶囊内镜推送入十二指肠降部操作方法的应用价值.方法 将行胶囊内镜检查的67例患者随机分为两组,研究组29例患者吞服胶囊后每20 min实时监测胶囊内镜在体内的运行位置,发现超过60 min胶囊内镜仍停留在胃内,则在无痛胃镜下用圈套器将胶囊内镜推送入十二指肠降部;对照组38例患者吞服胶囊后回病房,检查8~10 h之后,指示灯完全停止,此时拆卸胶囊内镜检查设备,检查结束.对比两组小肠完整检查成功率和胶囊在胃和小肠的停留时间.结果 研究组小肠完整检查成功率100%,对照组小肠完整检查成功率78.9%,两组比较差异有统计学意义(P<0.05);研究组患者胶囊内镜在胃内停留时间明显短于对照组,差异有统计学意义(P<0.05);研究组和对照组患者胶囊内镜在小肠内停留时间对比差异无统计学意义(P>0.05).结论 在无痛胃镜下使用圈套器辅助推送胶囊内镜进入十二指肠降部可以提高小肠完整检查的成功率.
Objective To eyaluate the application value of capsu[e endoscopy being pushed iuto descending duodenum with snare under painless gastroscopy. Methods A total of 67 patients were randomly divided into two groups : the study group ( n = 29) : every 20 minutes after swallowing capsules, in vivo real-time monitoring of the operating position was carried out, and if the capsule endoscopy was still found remained in the stomach 60 min later, the capsule would be pushed into descending duodenum with snare under painless gastroscopy; and the control group ( n = 38 ) : patients were sent back to the ward after swallowing capsules, capsule endoscopy equipment would be disassembled after the lights completely stopped 8 - 10 hours later. The rate of complete inspection of the small intestine, gastric emptying time, and small bowel transit time were compared between the two groups. Results The rate of complete inspection of the small intestine of the study group was 100%, and it was 78.9% in the control goup, the difference was statistically significant (P 〈 0.05). Gastric emptying time of the study group was significantly shorter than that of the control group, and the difference was statistically significant (P 〈 0.05) . Small bowel transit time were not significantly different(P 〉 0.05 ) between the two groups. Conclusion Capsule endoscopy being pushed into descending duodenum with snare under painless gastroscopy can improve the success rate of complete inspection of the small intestine.
