摘要 目的:探讨全胸腔镜手术(VATS)肺癌根治术的临床应用价值.方法:VATS行肺癌根治术21例,其中右上肺癌6例,右中肺癌2例,右下肺癌4例;左上肺癌5例,左下肺癌4例,并同时进行肺门及纵隔淋巴结清扫.结果:21例无一例中转进胸,手术时间90~250分min,术中出血量50~600 ml,术后胸腔闭式引流2~4 d.随访2~36个月,21例均成活.术后3个月、6个月、1年复查胸部CT、ECT均未发现明显复发和转移.结论:VATS肺癌根治术技术可靠,安全、有效,出血少,恢复快. Objective:To explore the clinical value of video-assisted thoracoscopic sugery (VATS) in the treatment of lung carcer. Methods:Among 21 patients with radical operation of lung cancer, right-npper lung cancer in 6 cases, right-middle lung cancer in 2 cases, right-low lung cancer in 4 cases;left-upper lung cancer in 5 cases and left-low lung cancer in 4 cases were found. The lymph nodes in hilus pulmonis and mediastinum of all patients were cleaned. Results:None was thoracotomy, the operation time was 90 to 250 minutes, the utes intraoperative blood loss was 50 to 600 ml and the closed drainage time was 2 to 4 days. Twenty-one patients lived after following up for 2 to 36 months. The recurrence and metastasis were not found by CT and ECT at 3,6 and 12 months after operation. Conclusions:VATS in the treatment of lung carcer is reliable, safe, fast recovering and less bleeding, but a little expensive.
