摘要
目的探讨人附睾蛋白4与肿瘤相关糖类抗原125(CA125)、肿瘤相关糖类抗原199(CA199)联合测定对卵巢癌的诊断价值。方法电化学发光全自动免疫分析仪检测96例卵巢癌患者的血清HE4与CA125、CA199,并与健康体检者作对照。结果卵巢癌患者及健康体检者HE4平均水平分别为102.2 pmol/L、57.3 pmol/L、37.3 pmol/L;阳性率分别为78.0%、7.0%、1.6%。卵巢癌患者及健康体检者CA125平均水平分别为204.9 U/L、34.7 U/L、18.7 U/L;阳性率分别为59.0%、14.8%、6.6%。卵巢癌者及健康体检者CA199平均水平分别为123.2 pmol/L、30.5 U/L、28.5 U/L;阳性率分别为50.0%、22.2%、6.6%。卵巢癌患者HE4、CA125、CA199的平均值均明显高于卵巢良性肿瘤患者及健康体检者(P<0.001)。HE4、CA125、CA199识别卵巢癌的ROC曲线下面积分别为94.4%、91.4%、91.1%;其诊断的敏感性分别为97.7%、95.6%、94.0%。特异性分别为91.1%、87.2%、86.4%。结论 HE4与CA125、CA199联合检测可明显提高卵巢癌诊断的灵敏度,对卵巢癌的早发现、早诊断具有重要的临床意义。
Objective To discuss the joint diagnostic value of human epididymal protein 4, tumor associated carbohydrate anti- gen 125 (CA125), tumor associated carbohydrate antigen (CA199) for ovarian cancer. Methods Electrochemil automatic immunoassay analyzer was used to detect serum HFA, CA125, CA199 in 96 ovarian patients, 54 benign tumor patients and 60 healthy controls, and the results were compared among the three groups. Results Serum HE4 levels were 102.2 pmol/L, 57.3 pmol/L and 37.3 pmol/L in ovarian cancer group, benign tumor group and control group, and the corresponding positive rates were 78.0%, 7.0% and 1.6% , respectively. While the CA125 levels were 204.9 U/L, 34.7 U/L, 18.7 U/L in turn, and the corresponding positive rate were 59.0% and 14.8% , 6.6% ; The average levels of CA199 were 123.2 U/L 30.5 U/L, 28.5 U/L in turn, and the corresponding positive rates were 50.0% , 22.2% and 6.6%. Mean values of HE4, CA125 and CA199 in patients with ovarian cancer were significantly higher than those in patients with ovarian benign tumor and healthy controls ( P 〈 0. 001 ). The area under ROC curve of HE4, CA 125 and CA 199 in ovarian cancer were 94.4%, 91.4% and 91.1% respectively; The diagnostic sensitivities were 97.7%, 95.6% and 94.0% respectively; The specificities were 91.1%, 87.2% and 86.4% respectively. Conclusion Joint detection of HFA and CA125, CA199 can obviously improve the diagnostic sensitivity of ovarian cancer, having important clinical significance in early detection and early diag- nosis of ovarian cancer.
