21例内窥镜采集大隐静脉行冠状动脉旁路移植术患者的术后护理 被引量:5

摘要 总结在冠状动脉旁路移植术(coronary artery bypass grafting,CABG)中,通过内窥镜采集大隐静脉的临床效果及护理要点。由于此方法开展时间短,对操作者技术要求高,取桥血管时间较长,为预防桥血管损伤而影响手术效果,对我科室21例采用内窥镜取大隐静脉行CABG术的患者进行循环系统监护,监测心电图ST段改变,重视心前区疼痛性质,警惕心肌梗死发生,控制心率、血压,增加冠状动脉血流,保证桥血管有效灌注,维护心脏功能;呼吸道护理,加强体疗排痰,警惕呼吸机相关性肺炎;出入量与电解质平衡护理;术后抗凝及手术切口的护理。全部患者经积极治疗及护理后好转。术后随诊3个月,无1例出现桥血管堵塞现象,均无心绞痛复发及心肌梗死。21例患者取大隐静脉伤口均一期愈合。 This paper summarized the clinical efficacy and nursing of Coronary Artery Bypass Grafting (CABG) collected by endoscopic great saphenous veins. Perioperative care on 21 cases undergoing CABG surgery collected by saphenous vein in endoscopy department was tracked which included circulatory system monitoring, respiratory care, discrepancy and electrolyte balance, postoperative anticoagulation and incision care. All patients turned well after active treatment and nursing, no bridge vascular blockage or recurrence of angina and myocardial infarction occurred in 3-month postoperative follow up. All 21 cases were in primary healing at the wound of great saphenous vein.
