摘要
综述了糖尿病的发病形势,将连续性护理模式应用到糖尿病的护理中,强调在社区中为糖尿病患者实施出院计划,包括疾病认识、自我监测、饮食指导、药物指导、运动指导、自我保健指导等等。认为将连续性护理模式应用到糖尿病患者的护理中,有利于糖尿病患者有效地控制血糖,降低并发症的发生率,减轻社会负担。
Summarizing the onset of Diabetes Mellitus,we plan to employ the continuous nursing method for diabetes patient care,emphasizing the application of the community diabetes discharge plan, including awareness of disease.self-monitoring, dietary guidance. medicine instructions.exercise instructions.self-care guiding and so on. We think we apply the continuous nursing method in diabetes patient care,through these initiatives,we hope to effectively control blood glucose in diabetic patients, reduce complications and reduce overall social burden.
