摘要
目的总结脑硬膜动脉血管融通(EDAS)术后脑膜中动脉的DSA影像的变化睛况,探讨这种变化的可能的原因。方法回顾胜分析131例行EDAS术治疗的烟雾病患者术前及术后血管造影的影像学资料。结果131例患者共249侧半球行EDAS手术治疗。术前造影可见54侧半球脑膜中动脉与脑表面血管存在沟通,术后脑血管造影见139侧存在脑膜中动脉扩张,并与颅内建立新的沟通。74侧同时进行了颞浅动脉与颌内动脉的超选造影,58侧中可见颞浅动脉与脑膜中动脉之间在脑表面形成相互沟通。结论烟雾病行EDAS术后,可见脑膜中动脉的主干及分支会扩张并与脑组织表面的血管形成良好的沟通.具体的作用机制尚不明确.但手术中对于脑膜中动脉主干及分支的保护非常必要。
Objective To summarize the change of middle meningeal artery after of encephalo-duro-arterio- synangiosis(EDAS) on Moyamoya disease and argue the probably reason of the changing. Methods Retrospective analysis of the DSA image before and after operations of EDAS on 131 patients with Mdoyamoya disease. Results Operation of EDAS were performed on 249 hemispheres of 131 patients with Moyamoya disease. Connection with vessels on cerebral of middle meningeal artery was found on 54 hemispheres. After the procedure, new connection was found on 139 hemispheres. We preformed super selection angiograph of superficial temporal artery and internal maxillary artery on 74 hemispheres, and new connection of them were found on 58 hemispheres. Condusiom The new connection of the stem and branches of middle meningeal artery with the vessels on brain tissues can be found after procedures of EDAS on patients with Mo^ainoya disease~ The mechanism is not clear. The protection of middle meningeal artery is very important during th~ procedure.
