摘要
目的:探讨妊娠妇女妊娠期间甲状腺功能变化及其与妊娠并发症、妊娠结局的相关性。方法:对2011年5月~2012年12月在沈阳铁西区、苏家屯区和新民市妇幼保健机构及妇产科门诊进行早孕检查并建立孕产妇保健手册的671l例孕妇,从妊娠12周~38周开始进行尿碘、妊娠期甲状腺疾病的流行病学检查,追踪妊娠并发症和妊娠结局,调查新生儿健康情况及甲状腺功能。结果:孕中期与孕晚期尿碘中位数比较,差异无统计学意义(Z=4.16,P〉0.05);孕晚期甲状腺激素指标异常的妇女尿碘值〉300μg/L者高于正常组(P〈0.05);孕妇妊娠结局及新生儿甲状腺功能与孕妇孕晚期尿碘水平存在相关性。结论:对孕妇开展尿碘监测防止不良妊娠结局及新生儿甲状腺功能异常具有一定的临床意义。
Objective: To explore the change of thyroid function of pregnant women and its correlation with pregnancy complica tions, pregnancy outcomes, and neonatal thyroid function. Methods: A total of 6 711 pregnant women who received pregnancy examination and registered pregnancy health caring profile at gynecological and obstetrical outpatient department and maternal and child health care hospi tals of Tiexi district and Sujiatun district of Shenyang city, Xinmin city from May 2011 to December 2012 were selected, their urine iodine levels were detected during 12 38 gestational weeks, an epidemiological investigation of thyroid diseases during pregnancy period was per formed, the pregnancy complications and outcomes were followed up, the neonatal health and thyroid function were surveyed. Results: There was no statistically signification difference in the median of urine iodine between the second trimester of pregnancy and the third trimes ter of pregnancy (Z = 4. 16, P 〉 0. 05 ) . The proportion of women with urine iodine 〉 300 μg/L among the women with abnormal thyroid hormones during the third trimester of pregnancy was high than that among the women with normal thyroid hormones ( P 〈 0. 05 ) . There was a correlation between pregnancy outcome of pregnant women, neonatal thyroid function and urine levels of pregnant women during the third trimester of pregnancy. Conclusion : Conducting urine iodine monitoring among pregnant women has a certain clinical significance in preven ting adverse pregnancy outcomes and neonatal thyroid dysfunction.
