十二指肠乳头小切开联合气囊扩张术在胆总管1cm以下结石治疗中的应用被引量：2

摘要目的探讨十二指肠乳头小切开联合气囊扩张术(EPBD)治疗胆总管结石的可行性。方法选择胆总管1 cm以下结石病人79例,先作选择性胆管逆性插入和胆管造影,确定结石大小,根据病人乳头大小和结石直径决定乳头括约肌小切开的范围。乳头切开后,插入取石气囊取胆总管切口位置扩张切口,再用取石网篮套取结石并将结石拉出胆总管。结果病人一次全部取出所有结石,各有一例病人发生胆道感染和一过性血淀粉酶升高,极少部分病人术中出现少于5 mL的出血和术后出现轻微腹胀、腹痛,未出现严重的出血、胰腺炎和穿孔等并发症。结论内镜下乳头括约肌小切开联合气囊扩张术治疗1.0 cm以下的胆总管结石安全可行。
