摘要
目的 探讨代谢综合征与臂踝脉搏波传导速度(ba-PWV)之间的关系.方法 选择2009年10月-2011年12月来我院体检的健康体检者3 200例为研究对象,其中男2 112例,女1 088例;年龄35-64岁.测量身高、体质量、血压;检测空腹血糖、血脂和ba-PWV.采用中华医学会糖尿病学分会(CDS)建议标准诊断代谢综合征.以ba-PWV<14 m/s为正常,14- m/s为轻度增快,16- m/s为中度增快,18- m/s为重度增快.对比分析代谢综合征组与非代谢综合征组的ba-PWV.结果 3 200例受试者中代谢综合征总检出率为11.5%(368例),其中男性检出率为14.6%(309/2 112)、女性检出率为5.4%(59/1 088).两组中35-、45-及55-岁患者ba-PWV间差异有统计学意义(P=0.000),其中55-岁者ba-PWV快于45-岁者,45-岁者快于35-岁者(P<0.05).代谢综合征组和非代谢综合组男性ba-PWV分别为(15.7±2.4)、(13.9±1.8) m/s;女性分别为(16.2±3.1) 、(13.2±2.1)m/s,组间差异有统计学意义(P<0.05).无论男女,代谢综合征组各年龄段ba-PWV均快于非代谢综合征组(P<0.05).代谢综合征组和非代谢综合组男性ba-PWV增快检出率分别为79.0%(244/309)和40.3%(726/1 803),女性分别为76.3%(45/59)和28.6%(294/1029),组间差异均有统计学意义(P<0.05).无论男女,代谢综合征组ba-PWV增快检出率均高于非代谢综合征组(P<0.05).结论 代谢综合征与动脉僵硬度密切相关,对存在心血管危险因素的中老年人应重视代谢综合征和脉搏波传导速度的检出和干预.
Objective To explore the relationship between metabolic syndrome (MS) and brachial ankle pulse wave velocity (ba -PWV) in healthy subjects. Methods A total 3 200 healthy subjects, 2 112 males, 1 088 females, aged 35 - 64 years, were enrolled in this study. Height, weight, and blood pressure (BP) were measured, fasting blood glucose (FBG), lipids, ba- PWV detected. The standards of MS proposed by Chinese Diabetes Society (CDS) of Chinese Medical Association. Taking ba -PWV 〈14 m/s as normal, 14 - m/s as slightly fast, 16 - m/s as moderately fast, 18 - m/s as se- verely fast. The ba - PWV of groups MS, non - MS was analyzed comparatively. Results In 3 200 subjects, MS total detec- tion rate was 11.5% (368 cases), there into 14. 6% (309/2 112) in males, 5.4% (59/1 088) in females. There was sig- nificant difference in ba - PWV between patients aged 35 - , 45 - , 55 - years in 2 groups ( P = 0. 000 ). ba - PWV was faster in patients aged 55 - years than in those aged 45 - years, faster in those aged 45 - years than in those aged 35 - years ( P 〈 0.05). The ba - PWVs of males in groups MS, non - MS were ( 15.7 ±2.4) m/s, ( 13.9 ±1.8) m/s; those of females were ( 16. 2 ±3. 1 ) m/s, ( 1 3. 2 -± 2. 1 ) m/s ; the difference was significant ( P 〈 0. 05 ). The ba - PWV was faster in MS group than in non - MS group in any age group ( P 〈 0. 05 ). The detection rates of fast ba - PWV of groups MS, non - MS were 79. 0% (244/309) and 40. 3% (76/1 803) in male, 76. 3% (45/59) and 28.6% (294/1 029) in female, respectively, the difference was significant ( P 〈 0. 05 ). The detection rates of fast ba - PWV were higher in male than female both in MS group and non - MS group (P 〈 0. 05). Conclusion MS is closely associated with arterial stiffness. Detection of ba - PWV, MS and intervention should be paid attention to in middle -aged, elderly patients with cardiovascular risk factors.
