分期手术治疗严重开放性踝关节骨折的疗效观察 被引量：14

摘要 目的观察分期手术治疗严重开放性踝关节骨折的疗效，探讨严重开放性踝关节骨折的治疗要点。方法采用分期手术治疗19例严重开放性踝关节骨折，一期清创缝合后采用负压封闭引流技术（VSD）冲洗灌注引流并行有限内固定，二期行切开复位内固定术。术后记录住院时间、AOFAS评分、骨折愈合时间以及并发症发生情况。结果19例获得随访平均13（10～17）个月，住院时间平均16（12～20）d，AOFAS评分平均83（70．92）分，骨折愈合时间平均13（10～16）周，一期术后4例伤口发生浅表感染，给予抗生素治疗后愈合；3例出现皮缘发黑，之后逐渐愈合。结论对严重开放性踝关节骨折分期手术治疗可取得满意的临床效果，早期应用抗生素及彻底清创、VSD冲洗灌注引流、解剖复位及后期功能康复训练是治疗成功的关键。
