摘要
目的 制备兔烫伤早期多器官功能衰竭 (MOF)模型 .方法 兔乙醚吸入法全身麻醉后 ,以 95℃水烫伤其背部及躯干两侧 30 s,造成 30 %体表面积 III度烫伤 (III度 30 %TBSA) .烫伤后 48h,用戊巴比妥钠耳缘静脉注射 (30 m g·kg- 1 )麻醉兔 ,检测假烫对照组及烫伤组 48h后的平均动脉血压、中心静脉压、心输出量 ,计算总外周阻力、心指数 ;测定肺泡灌洗液蛋白及血浆总蛋白 ,计算肺通透指数 ;测定动脉血氧分压、氧饱和度 ,测定血中肌酸磷酸激酶、谷丙转氨酶、尿素、肌酐、血小板等反映心、肺、肝、肾、血液功能的一系列指标 .结果 烫伤后 48h兔心、肝、肾功能下降达到衰竭标准 ,肺功能下降 ,总外周阻力上升 ,动物发生 MOF.结论 利用兔可制备烫伤早期 MOF模型 ;
AIM To establish model of rabbit multiple organ failure (MOF) in early postburns. METHODS All rabbits were anesthetized with ether inhalation. The dorsal skin of rabbits were immersed in 95℃ water (or 37℃ water) for 30 s. This procedure produced a full thickness burn of 30% total body surface area (TBSA) or sham burn. At 48 h postburn, all rabbits were anesthetized with intravenous injection of barbiturate (30 mg/kg). Mean arterial pressure (MAP), central venous pressure (CVP), cardiac output (CO) were monitored, and systemic vascular resistance (SVR) and cardiac index (CI) were calculated. Pulmonary alveolar perfusion protein (PAPP), plasma total protein (TP) were determined, and index of pulmonary permeability were calculated from (PAPP/TP)%. Partial pressure of oxygen of arterial blood (PaO 2), oxygen saturation of arterial blood (SaO 2) were measured. Levels of serum creatine phosphokinase (CK), glutamic pyruvic transaminase (GPT), urea (BU), creatinine (Cr) and blood platelet count (PLT) were detected. These data reflected separately the functions of heart, liver, lung, kidney and blood coagulation system. RESULTS Damages of functions of heart, liver and kidney achieved the criterion of MOF, the lungs function declined, but SVR increased. Animal developed MOF at 48 h postburns. CONCLUSION Rabbits model of MOF could be established in early postburns. MOF might develop when animal blood pressure remains normal.
