摘要
目的探讨层流手术室术后环境对甲状腺手术切口感染的影响,寻求适宜的消毒及自净时间,以降低切口感染发生风险。方法将我院2012年1月至6月227例连台行甲状腺切除手术的患者进行前瞻性调查,根据术后连台手术时间分为4组,分别为0、10、20、30min组,采样共计624例进行环境监测,将后2组监测结果与手术患者感染情况进行分析。结果 4组监测结果,空气培养及物体表面监测合格率各组差异均有统计学意义(P<0.05)。空气培养及物体表面监测菌落数差异有统计学意义(P均<0.05)。术后净化及消毒20min组手术患者感染率为8.80%,30min组手术患者感染率为1.96%,差异有统计学意义(P<0.05)。结论层流手术室术后消毒及自净时间30min满足要求,手术室环境与甲状腺切除手术患者发生切口感染关系密切,加强手术室环境监测与管理对控制医院感染有重要意义。
Objective To investigate the effect of postoperative environment on the surgical incision infec-tion of thyroid surgery in laminar flow operation rooms ,and to study the suitable time of disinfection and self-puri-fication ,in order to reduce the risk of incision infection .Methods Two hundreds and twenty-seven patients who performed thyroid resection operation from January to June in 2012 were prospectively investigated in our hospital . According to the operation time ,they were divided into four groups (0 ,10 ,20 ,30 minutes group ,respectively) . A total of 624 sampling cases were performed for environmental monitoring and the difference of the monitoring results and infection status between the last two groups of surgery patients was compared .Results The qualified rates of air culture and surface monitoring were significantly different between groups (P&lt;0.05) .The difference of colony counting of air culture and object surface monitoring was statistically significant (P&lt;0.05) .The infec-tion rate of 20 minutes and 30 minutes group were 8.80% and 1.96% ,respectively ,and there was a significant difference between them (P&lt;0.05) .Conclusion The time for 30 minutes of postoperative disinfection and self-purification could be appropriate in the laminar flow operating room .There is a close correlation between the envi-ronment of operating room and the resection incision infection of thyroid .It is important for the control of hospital infection to strengthen the environmental monitoring and management in operating rooms .
