预防依托咪酯诱发肌颤的研究进展 被引量:20

摘要 依托咪酯是一种短效的非巴比妥类催眠性静脉全麻药,起效快、对循环系统和呼吸影响小,尤其适合于血流动力学不稳定患者及老年患者的麻醉。但依托咪酯在全麻诱导期间常诱发肌颤产生,其发生率高达50%~80%,肌颤可导致肌纤维损伤、肌痛、血清钾升高以及饱胃患者反流误吸率升高,不仅会给手术操作造成不便,还会使患者术后感到不适,甚至危及患者生命。目前研究认为肌颤是不正常的神经肌肉活动,但其发生的确切机制仍不明确。如何减少依托咪酯诱发肌颤的产生是其应用的研究方向之一。近期研究表明肌松药、阿片类药物、右美托咪定、咪达唑仑、硫酸镁和依托咪酯预注等对肌颤的发生均有预防作用,本文将就依托咪酯诱发肌颤的机制及治疗药物进展做一综述。
