不同宫颈组织eIF-4A蛋白表达及其临床意义探讨 被引量:2

摘要 目的:探讨真核细胞起始因子eIF-4A在正常及不同级别病变宫颈组织中的表达及其临床意义。方法:选取青岛大学医学院附属医院妇科2011-01-01-2012-06-01行手术治疗或活检确诊的120例患者,其中低度鳞状上皮内病变组织(CINⅠ级)30例,高度鳞状上皮内病变组织(CINⅡ和Ⅲ级)30例及宫颈癌组织60例(51例鳞癌,9例非鳞癌)。另选取同期手术切除的30例正常宫颈组织作为对照。应用免疫组织化学方法测定不同宫颈组织中eIF-4A蛋白的表达水平,探讨其与宫颈癌临床病理参数之间的关系。结果:eIF-4A蛋白的阳性表达率在正常宫颈组织(0)、低度鳞状上皮内病变(46.7%)、高度鳞状上皮内病变(76.7%)及宫颈鳞状细胞癌组织(78.4%)中逐渐增高,差异有统计学意义,χ2=54.421,P<0.001;且趋势检验有统计学意义,χ2=47.111,P<0.001;但是高度鳞状上皮内病变组织和宫颈鳞状细胞癌组织中eIF-4A的阳性表达差异无统计学意义,χ2=0.034,P=0.854。eIF-4A蛋白表达与宫颈癌组织学类型无关,χ2=0.592,P=0.442;与宫颈癌的病理分级有关,χ2=6.161,P=0.046;与临床分期有关,χ2=7.515,P=0.006。结论:eIF-4A可能参与了宫颈病变发生和发展的过程,可能在宫颈病变的初始过程即发挥了作用。 OBJECTIVE: To explore the expression of elF-4A in cervical tissue and its clinical significance. METH- ODS: Pathologically confirmed cervical specimens,including 30 cases of CIN I , 30 cases of CIN II and Ⅲ , 60 cases of cervical intraepithelial cancer,and 30 normal cervical tissues,were selected from 2011-01-01 to 2012-06-01 in Department of Gynecology, Affiliated Hospital of Medical College of Qingdao University. Immunohistochemistry was used to detect the expression of eIF-4A in differential cervical tissues to explore its relationship with clinicopathological parameters between cervical cancers. RESULTS: The expression of elF-4A was not detected in normal cervical tissue. The expression level was gradually increased with increment of grading of epithelial lesions in the order of CIN I (46. 7%), CIN I andⅢ(76.7%) and SCC(78.4%). The difference among the four groups was statistically significant ((2 = 54. 421 ,P〈0. 001) and the trend test was statistically significant too (X2 = 47. 111, P〉0. 001), hut there was no significant difference be tween high-grade squamous intraepithelial lesions and cervical squamous cell carcinoma (X2 = 0. 034, P = 0. 854). The overexpression of eIF-4A was significantly related to the differentiation and clinical stage of tumor (x2= 6. 161, P= 0. 046;)C = 7. 515, P = 0. 006), but it was not significant correlated with the histopathological types (x2= 0. 592, P= 0. 442). CONCLUSION: The overexpression of eIF-4A in cervical cancer may play a role in the oncogenesis and malignant progression of cervical cancer.
