摘要
目的探讨Sweet综合征的临床特点,提高对本病的认识。方法回顾性分析21例Sweet综合征患者临床资料和随访资料。结果 21例(100%)患者均出现疼痛性红色斑块或结节,18例(85.7%)出现假性水疱,15例(71.4%)伴有体温升高,9例(42.9%)皮损位于面颈部,7例(33.3%)位于上肢和手部,3例(14.3%)位于躯干部,2例(9.5%)位于下肢。21例表皮层均完整,其中1例表皮层见中性粒细胞浸润,21例真皮层上中部均可见中性粒细胞浸润,其中3例并可见较多的淋巴细胞浸润,2例可见嗜酸性粒细胞浸润,17例可见真皮乳头水肿,4例伴有核尘,2例皮下组织也可见中性粒细胞浸润。19例系统应用糖皮质激素疗效确切。结论 Sweet综合征多见于中青年女性,以疼痛性红斑或结节、假性水疱伴发热为主要临床表现,皮损多发生于面颈部、躯干部及四肢部,可并发其它系统疾病,病理特点为真皮内以嗜中性粒细胞浸润为主。
Objective To investigate the clinical characteristics of sweet syndrome, and improve the understanding of this disease. Methods Clinical data and follow-up data of 21 patients with sweet syndrome were analyzed retrospec- tively. Results The 21 patients（100%） had painful erythematous plaques or nodules, there were false blisters in 18 pa- tients （85.7%）, and 15 patients （71.4%） accompanied with elevated body temperature, skin lesions were located in the face and neck of 9 patients （42.9%）, skin lesions were located in the upper limb and hand of 7 patients（33.3%）, skin lesions were located in the trunk of 3 patients（14.3%）, skin lesions were located in the lower limbs of 2 patients（9.5%）. Complete epidermal layer was shown in 21 patients, neutrophil infiltration was seen in epidermis in 1 patient, neu- trophil infiltration was seen in the upper middle part of the dermis in 21 patients, 3 patients among them were ob- served many lymphocytic infiltrations, and eosinophilic infiltrations were showed in 2 patients, dermal papilla edema was shown in 17 patients, it was accompanied with nuclear dust in 4 patients, and neutrophi] infiltrations were also found in subcutaneous tissue in 2 patients. 19 patients who used systemic glucocorticoids had definite curative effect. Conclusion There are a lot of sweet syndrome occurs in young and middle-aged women, the pain with lupus or nod- ules with fever are the main clinical manifestations, the lesions occurs in face and neck, trunk and extremities, it may be complicated by other diseases, the pathological characteristics are neutrophil infiltration in the dermis, it is an ef- fective therapies by using systemic corticosteroids.
