高浓度七氟烷用于小儿肛肠手术的麻醉效果观察 被引量:9

摘要 目的观察高浓度七氟烷全凭吸入麻醉用于小儿肛肠手术的临床麻醉效果。方法选择我院2012—2013年收治的需择期进行肛肠手术的患儿80例,通过查随机数字表法将患儿随机分为观察组和对照组各40例。观察组全凭七氟烷吸入麻醉。对照组行传统氯胺酮全凭静脉麻醉。比较两组患儿麻醉前(T0)、麻醉后1min(T1)、5min(T2)、手术开始(L)、手术结束(T4)、苏醒时(T5)的心率(HR)、血氧饱和度(SpO2)、呼吸次数(RR)、脑电双频指数(BIS),同时记录患儿诱导时间、睁眼时间、苏醒期躁动评分等指标。结果对照组患儿心率增快(P〈0.05),呼气末二氧化碳(EtCO2)明显高于观察组(P〈0.05),观察组术后苏醒时间、睁眼时间早于对照组(P〈0.05)。结论七氟烷全凭吸入麻醉用于小儿肛肠手术血液动力学指标较稳定,和氯胺酮麻醉比较具有诱导迅速和苏醒快而完全的优点。 Objective To observe the clinical anesthesia effect of inhalation anesthesia with high concentration sevoflurane in pediatric anorectal surgery. Methods Eighty children who had received the pediatric anorectal surgery during 2012 to 2013 in our hospital were randomly divided them into observation and control groups,40 cases in each group. The observation group received sevoflurane inhalation anesthesia while the control group received the traditional intravenous anesthesia with ketamine. The heart rate, blood oxygen saturation, respiratory rates, and Bispectral Index before anesthesia( To ), 1 minute after anesthesia( T1 ) ,5 minutes after anesthesia (T2 ), the surgery beginning( T3 ), the surgery finishing( T4 ), and awakening time (T5 )were compared between the two groups. At the same time, the children induction time, eye opening time, awakening period agitation score were also compared. Results The heart rate was faster and the end-tidal carbon dioxide partial pressure was higher in the control group than that in the observation group ( all P 〈 0. 05 ). The awakening time and eye opening time in the observation group were earlier than that in the control group( all P 〈 0. 05 ). Conclusion The hemodynamic indexes are stable during sevoflurane inhalation anesthesia for pediatric anorectal surgery. It has advantages of fast induction and quick awakening when compared with the ketamine anesthesia.
