摘要
目的探讨基于记忆规律结合实践计划-实施-检查-处理(PDCA)带教模式对提高手术室护生手卫生知识掌握情况的影响。方法选取2009年9月-2012年9月在医院手术室实习的240名护生随机分为观察组和对照组,每组120名,对照组护生按照常规带教模式进行手卫生知识教育,观察组护生进行基于记忆规律的PDCA循环带教模式,对所有护生洗手知识知晓情况、外科洗手知识掌握和外科洗手效果进行调查。结果观察组护生在无菌操作之后、接触不同的手术患者、戴手套前、脱手套后,接触到患者的体液血液分泌物后、从污染区到无菌区、侵入性操作前、后和下班前的洗手率分别为90.8%、85.0%、88.3%、85.0%、93.3%、94.2%、90.0%、85.8%和91.7%,均明显高于对照组,且差异有统计学意义(P<0.05);观察组护生的外科洗手法、洗手次数、洗手范围、洗手时间和洗手用具等知识的合格率分别为88.3%、93.3%、91.7%、90.0%和88.3%,均明显高于对照组,且差异有统计学意义(P<0.05);观察组护生的外科洗手合格率95.8%,明显高于对照组的81.7%,差异有统计学意义(P<0.05)。结论基于记忆规律的PDCA带教模式可以显著提高手术室护生手卫生意识,提高其外科洗手知识的掌握率。
OBJECTIYE To explore the effect of plan-do-check-processing (PDCA) mode based on memory rule on the nursing students" understanding of hand hygiene knowledge. METHODS A total of 240 nursing students, who took the internship in operating rooms from Sep 2009 to Sep 2012, were randomly divided into the observation group and the control group, with 120 in each, then the hand hygiene education was conducted in the control group according to conventional teaching mode, while the PDCA teaching mode based on the memory rule was applied in the observation group, and the awareness of hand hygiene knowledge, grasp of surgical hand washing knowledge, and effect of surgical hand washing were investigated. RESULTS In the observation group, the rate of hand washing was 90. 8% after the aseptic operation, 85. 0% before contacting the patients, 88. 3% before wearing gloves, 85. 0% after taking off gloves, 93. 3% after contacting the patients" body fluid, blood, or secretions, 94.2 % when transferring from contaminated zone to sterile area, 90.0% before the invasive operation, 85.8% after the invasive operation, 91.7% after work, significantly higher than those of the control group (P〈 0.05). The qualified rate of surgical hand washing method of the observation group was 88.3%, the frequency of hand washing 93.3%, the range of hand washing 91.7%, the hand washing time 90.0%, the hand washing apparatus 88.3%, significantly higher than those of the control group (P〈0.05). The qualified rate of surgical hand washing of the observation group was 95.8%, significantly higher than 81.7% of the control group (P〈0.05). CONCLUSION The PDCA teaching mode based on memory rule can significantly improve the awareness of hand hygiene of the nursing students in the operating room and facilitate the understanding of surgical hand washing knowledge.
