摘要
目的探讨128层螺旋CT血管成像(CTA)和灌注成像(CTPI)技术对胃周血管、胃癌血供和血流动力学评价及临床意义。方法 87例患者均经病理证实,62例采用体部灌注扫描序列对患者实行灌注扫描,分析血流量(BF)、血容量(BV)、平均通过时间(MTT)、表面通透性(PS)与胃癌的局部侵袭性和分化的关系及相关性;差异性采用单因素方差分析,相关性采用Spearman秩相关分析。25例行动态增强扫描,动脉期分析胃癌的位置及供血血管。结果 (1)25例胃癌患者19例腹腔干、肝固有动脉、胃左动脉、胃右动脉及胃网膜右动脉在容积再现(VR)上能够很清晰地显示出来,最大密度投影(MIP)显示细小血管较好,胃癌血供同肿瘤生长的位置有关,供应胃癌的血管基本都来自于胃的自身血管。(2)BF、BV、MTT、PS与T分期差异有统计学意义(P<0.05);MTT可反映胃癌大体分型的组织学特性;低分化组与中高分化组比较PS差异有统计学意义(P<0.05),与BV、BF、MTT、PS之间无明显相关性。结论 128层螺旋CTA可以清晰地显示胃的血供及肿瘤血管,并有利于从多个角度观察血管与肿瘤的关系;CTPI在一定程度上可以反映胃癌的局部侵袭性和分化程度。
Objective To discuss significance of 64 row helical CT angiography (CTA) and vascular perfusion imaging (CTPI) in evaluation of perigastric vein, blood supply and hemodynamic of gastric cancer. Methods 87 cases with gastric cancer confirmed by surgery or pathology were selected, the CT perfusion body tumor software, the CT perfusion parameters including blood volume (BV), blood flow (BF), mean transit time (MTF) and permeability surface (PS) with local invasion of gastric cancer and differentiation of relationship or correlation were analyzed. 25 cases received dynamic enhanced scanning and the location of the stomach cancer and arterial blood vessels were analyzed. Results ( 1 ) 19 patients in 25 cases of gastric cancer with celiac axis, proper hepatic artery, left gastric artery, the right gastric artery and right gastroepiploic artery could be clearly displayed on the VR, MIP showed better small blood vessels, stomach blood supply related to the location of the tumor growth and blood vessels that supply the gastric cancer was largely from their own blood vessels of the stomach. (2)There was statistically significant difference in BF, BV, MTT, PS and T staging; MTT could reflect the general classification of gastric cancer histological characteristics; There was statistically significant difference of PS in poorly differentiated group compared with middle and high differentiated group, PS had no significant correlation with BV, BF, MTT. Conclusion 64 row spiral CTA can clearly display the stomach and blood supply of tumor, CTPI can reflect local invasion and differentiation degree of gastric cancer to some extent.
