摘要
目的探讨分析口腔正畸金属固定矫治器光固化粘接托槽脱落的原因。方法对2010—2012年3年内在内蒙古医科大学附属医院口腔正畸科矫治的700例口腔错畸形患者(男286例,女414例)的临床资料进行回顾性分析,粘接MBT及ROTH直丝弓托槽13 500个,根据对年龄、牙位、牙列区段分组后托槽脱落的情况,分析其脱落的原因。结果经分析,总脱落托槽数目为2 084个,总的脱落率为15.44%。前磨牙区托槽脱落为1 192个(29.43%),前牙区托槽脱落为892个(9.44%)。其中下颌第二前磨牙脱落率最高,下颌侧切牙脱落率最低。后牙高于前牙,后牙区同名牙下颌高于上颌,前牙区同名牙上颌高于下颌。结论正畸托槽脱落的原因是多方面的,主要的原因大致可归纳为患者因素,医源性因素和牙齿的因素。
Objective To explore the causes of light-curing adhesive bracket expulsion of metal fixed appliances in orthodontic treatment. Methods A retrospective analysis was conducted about the clinical data of 700 patients(286 males,414 females) with malocclusion defor- mity who received orthodontic treatments at Orthodontics Department, Inner Mongolia Medical University Affiliated Hospital in three years from 2010 to 2012.13 500 orthodontic straight-wire brackets(MBT/ROTH) were bonded for sample patients, and the expulsion causes were analyzed according to age, teeth position, and tooth column section.Results The overall expulsion rate of brackets was 15.44 %(2 084 /13 500), including 29.43% (1 192/4 050)in premolar brackets, 9.44% (892/9 450)in anterior brackets.Among the cases, the expulsion rate of mandibular second premolar ranked the highest, while the expulsion rate of mandibular lateral incisor ranked the lowest.The expulsion rate of molar teeth was higher than labial teeth ; the expulsion rate of the same teeth of mandible in posterior region was higher than maxil- lary; and the expulsion rate of the same teeth of maxillary in anterior region was higher than mandible.Conclusion There are many factors related to bracket expulsion of fixed appliances in orthodontic treatment, which can be mainly attributed to patient factors, iatrogenic factors, and dental factors.
