儿童内八字足底压力特点及矫正效果探析 被引量:11

摘要 借助footscan足底压力测试系统、丹尼斯·布朗矫形鞋等仪器,对40名3~5岁双侧内八字步态儿童和20名正常步态儿童进行对比分析,旨在揭示内八字步态儿童步向角、足跟外翻角、足底峰值压强分布等足底压力特点;通过40名内八字儿童为期6个月的矫正实验,对比分析矫正前、后的足底压力特征,客观地了解和评价不同方案的矫正效果,为探讨标本兼治的内八字步态矫正方法提供依据。研究显示,内八字步态特征:1)儿童内八字步态按步向角可分为轻、中、重三个级别,即-10°<轻度<0°,-18°≤中度≤-10°,重度<-18°;2)与正常步态儿童相比,内八字儿童的步向角和足跟外翻角都存在非常显著性差异,内八字步态儿童Toe 1、M4、M3和HM足底峰值压强大且均有显著性差异;3)正常步态足前、中、后相对冲量的比值约1∶5∶2,而内八字步态约为1∶0.2∶0.2。矫正实验结果显示,1)以步向角是否在正常范围内为标准,6个月后,丹尼斯·布朗鞋的矫正成率为100%,仅改变不良动作的成功率为38.5%;2)与正常步态儿童相比,两种方案矫正成功后的内八字儿童,行走时的步向角、足跟外翻角、部分区域峰值压强及相对冲量仍然有显著性差异。结论:1)内八字步态伴随不同程度的足跟外翻;2)相对于正常步态儿童,内八字步态儿童足底各部位峰值压强、相对冲量分布不均匀,不合理,有导致足底骨肌系统慢性损伤和不适症的可能;3)相对来说,矫形鞋可以促进儿童内八字步态在较短时间内,更快的趋于正常范围,但矫形鞋和仅改变不良动作的方法均不是理想的矫正方案。建议:对儿童内八字生物力学特征进行更深层次的综合研究,从肌肉力量训练角度寻找标本兼治的矫正方法。 The aims of this study were to explore the plantar pressure characteristics of toe-in children with Rscan plantar pressure system,and to compare and evaluate the effects of different treatment.20normal gait children and 40bilateral toe-in children with the age of 3-5years were recruited.All toe-in children were required to sit without"W "and"cross-leg"position and to sleep without prone position.The difference between two randomly divided groups was that one group wore Dannis-Brown corrective shoes and the other did not.The results showed 1)Toe-in gait of children can be classified as three levels:mild(0°^-10°),moderate(-10°^-18°)and sever(more than-18°),2)There were significant differences between normal and toe-in gait children in progressive angle,heel valgus angle,regional Maximum plantar pressure and impulse,3)The progressive angle of Dannis-Brown group changed faster compared with No-Dannis-Brown group during the whole corrective period,4)Comparing with normal gait children,the seemingly normal gait children after 6 months still had statistical differences on progressive angle,heel valgus angle,regional Max pressure and impulse.Based on this research,the conclusion had been made that neither corrective shoe nor changing harmful position was ideally corrective method.It suggested that further comprehensive biomechanical study should be made to find corrective methods from muscle strength training.
