摘要
目的探讨护理干预在预防门脉高压脾切除术后门静脉血栓形成中的应用效果。方法采用目标性监测方法,进行综合护理干预:术前加强心理护理,完善各项检查,重视营养支持,严格肠道管理;术后严密观察病情,加强疼痛管理,重视健康指导,合理检查用药。对实施综合护理干预前后门脉高压脾切除术后患者门静脉血栓的发生率、平均住院天数、人均费用支出进行统计分析。结果实施护理干预后,门静脉血栓的发生率明显下降,由实施前的51.1%下降到实施后的23.4%(P<0.05);平均住院天数,由实施前的(17.34±6.51)d下降到实施后的(13.78±4.37)d(P<0.001);人均费用支出由实施前的(3.3±0.4)万元下降到实施后的(2.9±0.2)万元(P<0.01)。结论综合护理干预能有效降低门脉高压脾切除术后门静脉血栓的发生率,加快病床周转率,减轻患者的经济负担。
Objective To study the nursing intervention on the prevention of portal vein thrombosis after portal hypertension splenectomy .Method Using targeted monitoring method ,the comprehensive nursing intervention was carried ,including strengthen the psychological nursing ,preoperative perfect various checks ,pay attention to nutri-tion support ,strict management of intestinal tract ,Postoperative close observation condition ,in order to strengthen the management of pain ,attach importance to health guidance ,reasonable check drug use .To implement compre-hensive nursing intervention for patients with portal vein thrombosis after splenectomy ,incidence of hypertension , average hospitalization days ,the per capita expense were analyzed .Result After nursing intervention ,the incidence of portal vein thrombosis was significantly reduced from 51 .1% before the implementation to 23 .4% after imple-mentation（P〈0 .05） .Before the implementation ,the average hospitalization days was reduced from （17 .34±6 .51） days to （13 .78 ± 4 .37） after the implementation （P〈0 .01） .Before the implementation of per capita spending by ten thousand yuan （3 .3±0 .4） to （2 .9±0 .2） after the implementation of ten thousand yuan （P〈0 .01） .Conclusion Comprehensive nursing intervention can effectively reduce the incidence of portal vein thrombosis after portal hyper-tension splenectomy ,and to speed up the sickbed turnover rate ,reduce the economic burden of patients ,clinical sig-nificance .
