摘要
目的探讨"十"字形半紧闭式高频喷射通气模式应用于小儿先天性唇腭裂整复术麻醉中的效果。方法择期行先天性唇腭裂整复术患儿40例,按随机数字表法分为观察组20例和对照组20例。观察组采用"十"字形衔接管,将高频通气半紧闭式呼吸回路系统应用于小儿先天性唇腭裂畸形整复术麻醉中。对照组术中采用紧闭式常频通气。观察两组气道峰压及血气分析。结果两组均顺利完成手术,均未发生缺氧、呛咳、躁动、支气管痉挛、喉痉挛等不良反应;观察组气道峰压明显低于对照组(P<0.05);两组血氧饱和度、pH值、动脉血氧分压、动脉血二氧化碳分压比较,差异均无统计学意义(P>0.05)。结论 "十"字形半紧闭式高频通气应用于小儿先天性唇腭裂畸形整复术麻醉中的呼吸管理,可有效提高手术安全性。
Objective To explore the effect of cross-shaped half-closed type of high frequency jet ventilation applied to anesthesia for children with congenital cleft lip and palate undergoing palate surgery .Methods Forty children with congenital cleft lip and palate undergoing elective palate surgery were divided into the observation group of 20 cases and control group of 20 cases according to the random number table .The observation group used the cross-shaped bridging tube,and the high frequency ventilation half-closed circuit breathing system was used in the anesthesia for children with congenital cleft lip and palate undergoing palate surgery .The ventilation of the constant normal breathing machine was used in the control group.The observation on airway peak pressure and blood gas analysis were conducted in two groups .Results The operations were successfully completed in two groups ,there were no hypoxia,choking cough,restlessness,bronchos-pasm,laryngeal spasm,and other adverse reactions .The airway peak pressure of observation group was significantly lower than the control(P〈0.05).There was no significant difference in blood oxygen saturation ,pH,arterial blood oxygen partial pressure and arterial blood CO2 partial pressure between two groups(P〉0.05).Conclusion The cross-shaped half-closed type of high frequency jet ventilation applied to anesthesia for children with congenital cleft lip and palate undergoing palate surgery could effectively improve operation safety .
