摘要
目的探讨血清基质金属蛋白酶-9(MMP-9)对冠心病患者缺血再灌注治疗后早期心功能评价的价值。方法用夹心酶联免疫吸附法检测81例均行冠状动脉造影术(CAG)检查的冠心病患者术前1 d,术后1 d、3 d、5 d血清MMP-9、N-BNP水平,其中单纯CAG组30例(对照组),择期PCI组25例,急诊PCI组26例(均为AMI);心脏超声定量测定心脏结构以及功能指标。结果各组间比较,急诊PCI组血清MMP-9水平在术前1 d、术后1 d和3 d与对照组比较明显升高;择期PCI组血清MMP-9水平在术后1 d、术后3 d与对照组比较明显升高;急诊PCI组血清MMP-9水平在术后3 d与择期PCI组比较明显升高。急诊PCI组内各时间点比较,术后3 d血清MMP-9、N-BNP水平较术前1 d明显升高。上述差异均有统计学意义(P<0.05)。急诊PCI组术后3 d血清MMP-9与△VE存在负相关(r=-0.555,P=0.04),与N-BNP存在正相关(r=0.56,P=0.04),差异有统计学意义。结论血清MMP-9与N-BNP、△VE有良好的相关性,可以作为冠心病缺血再灌注治疗中对早期心功能进行评价的指标。
Objective To investigate the serum MMP-9 in coronary ischemia reperfusion therapy on early heart function to evaluate the application value. Methods With sandwich enzyme-linked immunosorbent assay for the detection of all 81 cases underwent CAG examination in patients with coronary heart disease before operation l day, postoperative 1 day, 3 day ,5 day serum MMP-9, N-BNP levels,in which only 30 cases in group CAG( control group) , delayed PCI group 25 ca- ses ,26 cases of emergency PCI group (all AMI);cardiac ultrasound quantitative determination of cardiac structure and function. Results Emergency PCI group serum MMP-9 levels compared with the control group increased significantly in preoperative 1 day, postoperative 1 day and 3 day. Delayed PCI group serum MMP-9 levels compared with the control group increased significantly in postoperative 1 day and 3 day. Emergency PCI group serum MMP-9 levels compared with delayed PCI group markedly elevated in the postoperative 3 day. Emergency PCI group the postoperative 3 day serum MMP-9, N-BNP levels were significantly elevated preoperative 1 day. The differences were statistically significant (P 〈 0.05). Emergency PCI group the postoperative 3day serum MMP-9 levels and AVE had negative correlation (r = -O. 555 ,P = O. 04 ), and N-BNP had a positive correlation (r = O. 56, P = 0.04 ), the difference was statistically signifi- cant. Conclusion The serum MMP-9 and N-BNP levels, AVE had good correlation. The serum MMP-9 levels can be used as a coronary isehemia reperfusion therapy on early cardiac function evaluation index.
