应用高频超声分型诊断臀肌挛缩症 被引量:5

High-frequency ultrasound in typing and diagnosis of gluteal muscle contracture

摘要 目的探讨不同类型臀肌挛缩症(GMC)的声像图特征及其临床应用价值。方法对193例经手术证实的臀肌挛缩症患者(共344侧患肢),术前行高频超声检查,观察臀大、中、小肌等臀部肌肉及髂胫束、结合部的超声表现,根据肌挛缩范围分为轻、中、重、极重4种类型,并与手术结果对照。结果超声诊断轻、中、重、极重4种类型GMC的敏感度分别为:100%(73/73)、77.31%(92/119)、69.89%(65/93)、55.93%(33/59),阳性预测值分别为78.49%(73/93)、76.67%(92/120)、68.42%(65/95)、91.67%(33/36)。超声与手术结果一致性检验的Kappa值为0.677(P<0.01)。挛缩臀肌的声像图表现为局部肌肉轮廓不清,肌纹理模糊、消失,严重者肌肉呈结节或团块状改变,肌肉萎缩变薄,肌肉间分界不清。结论应用高频超声可根据声像图特征对GMC患者进行诊断与分型,有助于临床选择治疗方案。 Objective To explore the ultrasonic appearances of different types of gluteal muscle contracture (GMC) and their clinical application value for typing and diagnosis. Methods One hundred and ninety-three patients with GMC (344 sides) confirmed by surgery were examined with high frequency ultrasound, and the imaging characteristics buttock mus- cles and iliotibial tract (ITT) were observed. Then GMC were divided into light type, medium type, heavy type and ex tremely heavy type according to the scope of contracture, and the results compared with surgical findings. Results The sensitivity of ultrasound for four types of GMC was 100%(73/73), 77.31% (92/119), 69.89% (65/93) and 55.93% (33/59), and the positive predictive value was 78.49% (73/93), 76. 67% (92/120), 68.42%(65/95) and 91.67% (33/ 36), respectively. There was good consistency between ultrasonic and surgical results (Kappa=0.677, P〈 0.01 ). The ultrasonic appearances of contracture gluteus included unclear or disappeared contour with local muscle texture fuzzy. With crumb structure intramuscularly, the muscles became thin, and the boundaries were unclear. Conclusion High frequency ultrasound is useful in diagnosing different types and guiding treatment of GMC.
