摘要
【目的】探讨血清脂蛋白a(LPa)、脑利钠肽(pro-BNP)、肌钙蛋白I(TnI)及总胆红素(TBIL)与慢性心力衰竭患者预后的关系。【方法】选取2012年1月至2013年4月于本院进行诊治的94例慢性心衰患者设为观察组,并以94名体检的健康人员设为对照组,将两组患者的血清LPa、pro-BNP、TnI及 TBIL水平进行比较,同时对观察组中不同纽约心功能分级(NYHA分级)患者的检测水平进行比较,分析各指标与心衰的关系。【结果】观察组血清LPa、pro-BNP、TnI及TBIL水平均高于对照组( P <0.05);观察组中NYHA分级高者各指标水平高于分级低者( P<0.05);经Logistic回归分析显示血清LPa、pro-BNP、TnI及TBIL均与疾病有密切的关系( P <0.05)。【结论】血清LPa、pro-BNP、TnI及 TBIL在慢性心衰患者中较对照组明显升高,且与NYHA分级关系密切,可在一定程度上反映疾病的严重程度,据此认为可作为判断疾病预后的指标,具有临床应用价值。
[Objective]To explore the correlation of serum lipoprotein (a) ,pro-BNP ,troponin I and total bilirubin with the prognosis of patients with chronic heart failure .[Methods] A total of 94 patients diagnosed as chronic heart failure in our hospital from Jan .2012 to April 2013 were enrolled as observation group ,and 94 healthy people were included as control group .Serum levels of lipoprotein(a) ,pro-BNP ,troponin I and to-tal bilirubin of two groups were measured and compared .The NYHA classification was applied to evaluate heart function of patients with chronic heart failure ,and the correlation between the above four serum biomar-kers and heart failure was analyzed .[Results]Serum levels of lipoprotein (a) ,pro-BNP ,troponin I and total bilirubin in observation group were higher than those in control group ( P 〈0 .05) .In observation group ,the above indexes of patients with high grade NYHA were higher than those of patients with low grade NYHA ( P〈0 .05) .Logistic regression analysis showed that serum levels of lipoprotein (a) ,pro-BNP ,troponin I and to-tal bilirubin were closely related with the disease ( P 〈0 .05) .[Conclusion]Compared with control group ,ser-um lipoprotein(a) ,pro-BNP ,troponin I and total bilirubin in patients with chronic heart failure are obviously increased and closely correlated with NYHA class .These biomarkers can reflect the severity of the disease , which suggests that they can be used as the indicators for assessing the prognosis of the disease ,and have clin-ical application value .
