微创跟腱吻合器治疗急性闭合性跟腱断裂术后并发症的Meta分析 被引量:2

摘要 目的通过Meta分析评价微创跟腱吻合器治疗急性闭合性跟腱断裂术后并发症。方法计算机检索数据库MEDLINE(1966年-2013年11月)、PUBMED(1966年-2013年8月)、EMBASE(1966年-2013年11月)、Cochrane图书馆(2013年第3期)、中国生物医学光盘数据库(1978年-2013年11月)、中国生物医学文献数据库(1978年-2013年11月)和维普中文科技期刊数据库(1978年-2013年11月)中关于微创跟腱吻合器(试验组)与传统切开吻合术(对照组)治疗急性闭合性跟腱断裂的临床随机对照试验,并查阅所有检出文献和相关综述的参考文献作为补充资料,截止到2013年11月。采用RevMan 5.1软件进行统计分析,试验组与对照组术后再断裂、切口感染、神经损伤和深静脉血栓发生率采用风险差异评价。结果共纳入高质量前瞻性随机对照试验4篇。Meta分析结果显示,与传统切开术比较,微创跟腱吻合器能够减少术后感染发生率(RD=-0.09,95%CI-0.16^-0.01,P=0.02),而再断裂(RD=-0.01,95%CI—0.04~0.06,P=0.65)、神经损伤(RD=-0.00,95%CI—0.04~0.04,P=1.00)和深静脉血栓(RD=-0.02,95%CI-0.09~0.05,P=0.57)的发生率差异均无统计学意义(P>0.05)。结论微创跟腱吻合器能够减少急性闭合性跟腱断裂术后感染发生率,不增加再断裂、神经损伤和深静脉血栓的发生风险。 Objective To evaluate the minimally invasive treatment of acute achilles tendon achilles tendon rupture closed postoperative complications with Meta-analysis.Methods Computer searched the database MEDLINE (1966-Nov, 2013), PUBMED (1966-Aug,2013), EMBASE (1966-Nov,2013), Cochrane Library (2013 Section 3), Chinese Bio-medical CD-ROM database (1978-Nov,2013), Chinese Biomedical literature Database (1978-Nov,2013) and VIP Chi-nese Scientific Journals database (1978-Nov,2013) about minimally invasive achilles tendon stapler (test group) and the traditional incision anastomosis ( control group ) in the treatment of acute achilles tendon rupture closed randomized controlled clinical trials, and access to all relevant documents and review detected references as supplementary information , until Novem-ber 2013.Using RevMan 5.1 software for statistical analysis , test group and the control group were re-fracture, wound infec-tion, nerve damage and the incidence of deep vein thrombosis were evaluated by using risk difference assessment .Results 4 high-quality prospective randomized controlled trials were included .Meta-analysis showed that , compared with the traditional incision, minimally invasive Achilles tendon stapler can reduce the incidence of postoperative infection (RD =-0.09, 95%CI -0.16 --0.01, P =0.02), and then break (RD =-0.01, 95%CI -0.04 -0.06, P =0.65), nerve damage (RD =-0.00, 95%CI -0.04 -0.04, P =1.00) and deep vein thrombosis (RD =-0.02, 95%CI -0.09-0.05, P =0.57) incidence difference was not statistically significant .Conclusion Minimally invasive stapler can reduce acute A-chilles tendon rupture closed the incidence of postoperative infection , fracture, the risk of nerve damage and deep vein throm-bosis do not increase again .
