远端缺血预处理减轻脂肪肝大鼠肝缺血再灌注损伤 被引量:2

摘要 目的探讨远端缺血预处理对脂肪肝大鼠肝缺血再灌注(I/R)损伤的保护作用机制。方法用高脂饲料制备大鼠脂肪肝模型后,将大鼠随机分成4组:假手术(S)组,缺血再灌注(I/R)组,缺血预处理(RIPC)组,RIPC+I/R组,每组8只。肝I/R模型制作方法:阻断入肝血流40 min,再灌注2 h;RIPC诱导方法:使用止血带使单后肢缺血5 min,再灌注5 min,反复6次处理;RIPC+I/R诱导方法:使用止血带使单后肢缺血5 min,再灌注5 min,反复6次处理后立即行I/R。所有缺血大鼠仅造成约70%肝缺血。取血用于血清谷丙转氨酶(ALT)、血清谷草转氨酶(AST)检测。切取肝组织用于测定热休克蛋白70(HSP70)的表达及观察显微结构的变化。结果与S组和RIPC组比较,其余各组ALT、AST明显升高(P<0.05),S组和RIPC组间相比无统计学意义(P>0.05),RIPC+I/R组明显低于I/R组(P<0.05);与S组和RIPC组比较,其余各组在脂肪变性基础上均出现不同程度水肿、变性及炎细胞浸润,但RIPC+IR组改变明显轻于I/R组;与S组比较,其余各组HSP70表达量均明显增加(P<0.05),RIPC+IR组HSP70表达量明显高于I/R组(P<0.05)。结论远端缺血预处理对脂肪肝缺血再灌注损伤有保护作用,其机制可能与诱导保护性蛋白HSP70有关。 Objective To investigate the protective effect of remote ischemic preconditioning against liverischemia/reperfusion (I/R) injury in rats with fatty liver and their possible mechanisms. Methods The model of fatty rat was induced by feeding high-fat diet. Subsequently, the fatty rats were randomly divided into four groups: sham group (S group), hepatic I/R group, RIPC group, RIPC+hepatic I/R group, with 8 rats in each group. The hepatic I/R model was produced by occlusion of hepatic inflow for 40 min and then reperfusion for 2 h, RIPC was induced by 6 cycles of 5 min occlusion and 5 min reperfusion of the unilateral hind limb using a tourniquet, and RIPC+I/R was elicited by 6 cycles of 5 min occlusion and 5 min reperfusion of the unilateral hind limb using a tourniquet followed by the I/R. All ischemia rats were only subjected to 70% liver ischemia. Finally, blood and liver samples were obtained to determine the activity of ALT, AST, the expressions of HSP70 protein, and pathology. Results Compared with S group and RIPC group, all other groups had higher levels of AST and ALT (P〈0.05), while these two indices had no significant differences between S group and RIPC group (P〉0.05), but those in RIPC+I/R group were obviously lower than that in I/R group (P〈0.05). Compared with S group and RIPC group, all other groups presented hydrops, degeneration and inflammatory cell infiltration of varying degrees on the basis of fatty degeneration, but the pathological changes in RIPC+UR group were obviously milder than those in I/R group. Compared with S group, the HSP70 expressions in liver tissues of other groups all significantly increased (all P〈0.05), but the HSP70 expression in RIPC+I/R group were significantly higher than that in I/R group (P〈0.05). Conclusion RIPC has protective effect against I/R injury of the hepatic adipose infiltration. The protective effects possibly are due to induction of protein HSP70 expression.
