摘要
目的探讨运动疗法促进脑梗死患者下肢运动功能恢复与PDGF-BB的相关性。方法随机将73例脑梗死患者分为脑梗死后非运动组与脑梗死后运动组,另选30例正常老人作为对照组。脑梗死后非运动组患者采取常规治疗;脑梗死后运动组患者在常规治疗的基础上,进行减重步行训练。分别在脑梗死后1 w、4 w、2 m、3 m 4个时点检测外周血PDGF-BB水平,采用FMA运动量表评估下肢运动功能恢复情况。结果脑梗死后运动组患者其PDGF-BB水平在检测的4个时点均较脑梗死后非运动组高(P<0.05);脑梗死后运动组患者其下肢FMA评分从4 w(1 m左右)开始,均明显高于脑梗死后非运动组(P<0.05),且2 m时评分高于4 w(P<0.05)。相关性分析显示,脑梗死后运动组下肢FMA评分与血PDGF-BB水平存在正相关性(r=0.67)。结论减重步行训练极有可能通过促进PDGF-BB的分泌帮助脑梗死后下肢运动功能的恢复。
Objective To explore the correlation of movement therapy and PDGF-BB to promote lower extremity motor function recovery in patient with cerebral infarction. Methods 73 cerebral infarction patients were randomly divided into two groups ,non-exercise group and exercise group, besides ,30 healthy elderly were selected as control. Patients in non-exercise group took conventional therapy,exercise group patients took the weight support treadmill training based on the eonveentional therapy. Plasma level of PDGF-BB was dected at 4 time point( 1 w,4 w,2 m,3 m ) respectively,and the low extremity motor function re- covery was evaluated by FMA scale. Result PDGF-BB plasma level in patients with exercise group was higher than that in nonexercise group(P 〈0.05);low extremity FMA score in paitents with exercise group was higher than that in non-exercise group from 4 w(P〈0.05) ,and the level at 2 m was higher than 2 w(P 〈0.05). By corelation analysis,there was a positve correlaton between low extremity FMA score and plasma PDGF-BB level in paitents with exercise group. Conclusions Weight support treadmill training may help the lower extremity motor function recovery by promoting the secretion of PDGF-BB.
