摘要
目的脑源性神经营养因子(brain-derived neurotrophic factor,BDNF)在血管性痴呆(vascular dementia,VD)发生和病程变化过程中所起的作用受到越来越广泛的关注,但有关血管性痴呆与外周血BDNF水平关系的研究结果不尽相同。本研究通过Meta分析的方法,对血管性痴呆与外周血BDNF水平的关联性进行评价,探讨外周血BDNF水平变化对于血管性痴呆的临床意义。方法计算机检索Pubmed、Embase、中国期刊网(CNKI)、万方数据库(WANFANG DATA)、中国科技期刊数据库(VIP),并辅以文献追溯等方法收集2013年8月以前国内外公开发表的关于血管性痴呆患者外周血BDNF水平的病例对照研究文献,采用Revman 4.2软件进行Meta分析,分析的主要结局指标是病例组与对照组间合并的标准化均数差(standardized mean difference,SMD)。结果共纳入5篇合格文献,累计243例病例和317例对照。血管性痴呆患者外周血BDNF水平显著低于对照组,其合并SMD值及其95%置信区间为-0.50(-0.68,-0.33)。结论 BDNF是血管性痴呆潜在的一个生物标记分子,但其病因关联性仍有待于在人群中开展大样本、前瞻性对照研究。
Objective There was increasing interest in the role of brain-derived neurotrophic factor(BDNF) in the onset and course of vascular dementia (VD) , but there were conflicting reports about peripheral blood levels of BDNF in pa- tients with VD. To conduct a systematic review of the relationship between peripheral blood levels of BDNF with VD by Meta-analysis, and to investigate the clinical significance of BDNF from peripheral blood in patients with VD. Methods Case-control studies about the peripheral blood level of BDNF in VD published at home and abroad were searched in Pubmed, Embase, CNKI, WANFANG DATA, and VIP. The method of tracing back was also used to found relevant arti- cles. Revman 4.2 software was used to conduct the Meta-analysis, the main outcome was the pooled standardized mean difference (SMD) between cases and controls. Results 5 articles with a total of 243 patients and 317 healthy volunteers were involved in analysis. The BDNF levels in VD were significant lower than controls. The pooled SMD and the 95% CI were - 0. 50 ( - 0. 68, - 0. 33 ). Conclusion BDNF was a potential biomarker to VD, but the etiologic relevance be- tween VD and BDNF needed higher-quality prospective controlled studies.
