摘要
目的用中医体质分类方法,比较痰湿体质与非痰湿体质高血压患者心血管功能的差异,并寻找在遗传方面的生物标志。方法 508例高血压病患者(痰湿质组264例,非痰湿质组244例)以左心室重量指数(LVMI)和颈动脉内膜-中膜厚度(IMT)为指标进行比较分析,并选择瘦素受体基因(Lepr)RS1137101基因多态性进行相关性分析。结果痰湿质组和非痰湿质组高血压患者IMT分别为(0.65±0.01)mm和(0.62±0.01)mm(P=0.019);痰湿质组高血压患者RS1137101基因AA、AG和GG的分布分别占1.5%、12.5%和80.6%,非痰湿组高血压患者分别占4.9%、13.9%和81.1%,A等位基因和G等位基因两组分别占8.0%、92.0%和11.9%、88.1%(P>0.05)。痰湿质组高血压患者的RS1137101AA型和GG型在IMT值比非痰湿质组患者明显升高,分别为(0.768±0.035)mm、(0.626±0.020)mm和(0.647±0.012)mm、(0.611±0.013)mm(P=0.017,0.039),而LVMI值两组无统计学意义。结论在痰湿质高血压患者中瘦素基因RS1137101的AA型和GG型与IMT厚度相关。
Objective To identify the cardiovascular risk factors and genetic markers in hypertension patients with dampnessphlegm constitution(DPC).Methods A total of 508hypertension patients were involved in the present study,among which 264were DPC and 244were non-DPC.The cardiovascular function was estimated by left ventricular mass indez(LVMI)and carotid intimamedia thickness(IMT).In addition,the association of RS1137101in the leptin gene and DPC was also tested.Results The IMT of DPC group(0.65±0.01mm)was higher than the non-DPC group(0.62±0.01mm,P=0.019).The frequency of AA,AG,and GG of RS1137101was 1.5%,12.5%,and 80.6%respectively in DPC group and that was 4.9%,13.9%,and 81.1%respectively in non DPC group.The frequency of A and G allele was 8.0% and 92.0%in DPC group,and that was 11.9% and 88.1%in non DPC group.The distribution of both genotype and alleles was not significantly different in two groups(P>0.05).The DPC hypertension patients with RS1137101-AA(0.768±0.035mm)and-GG(0.647±0.012mm)showed higher IMT than that in non-DPC group(AA:0.626±0.020mm,P=0.017;GG:0.611±0.013mm P=0.039).There was no significant difference in LVMI between two groups(P>0.05).Conclusion RS1137101-AA and-GG were associated with IMT in DPC hypertension patients.
