股骨近端防旋髓内钉治疗老年股骨粗隆间骨折临床效果探讨 被引量:3

摘要 目的:探讨股骨近端防旋髓内钉(PFNA)治疗老年股骨粗隆间骨折的临床效果。方法选择2008年1月~2012年1月在漯河市郾城区中医院接受治疗的股骨粗隆间骨折患者141例。按照自愿原则分成2组,采取PFNA(PFNA组)手术治疗股骨粗隆间骨折71例,采取动力髋螺钉(DHS组)手术70例,观察临床疗效和并发症。结果 PFNA组优良率(88.7%)和DHS组(82.9%)比较,差异无统计学意义。PFNA组手术时间[(56.5±12.4)min]和骨折愈合时间[(10.3±1.7)周]明显短于DHS组[(79.3±13.50 min;(14.6±1.8)周](P〈0.05),出血量[(121.5±20.7)mL]和术后大腿疼痛发生率(11.3%)明显低于DHS组[(238.8±69.3)mL、15.7%](P〈0.05),2组住院时间比较差异无统计学意义。PFNA组并发症发生率(1.4%)明显低于DHS组(11.4%)(P〈0.05)。结论PFNA治疗老年股骨粗隆间骨折创伤小、疗效高,安全,并发症低。
