2013年河南省60所中医院医院感染现患率调查分析 被引量:9

Survey of prevalence rates of nosocomial infections in 60 traditional Chinese medicine hospitals of Henan province in 2013

摘要 目的了解河南省中医系统医院感染现状,为控制医院感染、提高医疗服务质量提供决策依据。方法按照原卫生部《医院感染监测规范》的要求,采用查阅住院病历和床旁调查的方法,于2013年5月20日在河南省60所中医院开展医院感染现患率调查,借助全国医院感染监控管理培训基地的网络直报系统收集数据。结果 60所中医院应查18 112例,实查18 031例,实查率99.55%;医院感染平均现患率为2.21%,综合ICU医院感染现患率最高,为23.68%;Ⅰ类手术切口患者手术部位医院感染平均现患率为0.74%;医院感染现患率按科室排列由高到低为:综合ICU、儿科新生儿组、神经外科、胸外科、血液病科、肿瘤科、神经内科、内分泌科、普外科、消化科;不同规模医院医院感染现患率差异无统计学意义;所有调查医院抗菌药物使用率平均为35.24%,不同规模医院抗菌药物使用率差异有统计学意义(P<0.05);除儿科及综合ICU外,科室抗菌药物使用率前十位均为外科。结论中医系统内综合ICU为医院感染高危科室;医院规模越大,抗菌药物使用率越低;中医系统医疗机构依然需要加强医院感染管理工作。 OBJECTIVE To investigate the prevalence of nosocomial infections in traditional Chinese hospitals of Henan province so as to provide guidance for control of the nosocomial infections and improve quality of medical service. METHODS According to the requirements of Guidelines for surveillance of nosocomial infections by the Ministry of Health, a survey of prevalence of nosocomial infections was conducted in 60 traditional Chinese hospitals of Henan province on May 20th 2013 by means of review of medical records and bedside investigation, and the data were collected with the use of network direct reporting system of training base of national nosocomial infection surveillance. RESULTS Totally 18 112 patients were expected to investigate, and 18 031 cases were actually inves- tigated with the actual investigation rate of 99. 55%. The mean prevalence rate of nosocomial infections was 2.21%; the prevalence rate of the nosoeomial infections was highest in the general ICU (23.68%); the mean prevalence rate of surgical site infections in the patients undergoing type I incision surgery was 0.74%. The or- der of the prevalence rate of the nosocomial infections, from high to low, were the general ICU, neonatal wards of department of pediatrics, department of neurosurgery, department of thoracic surgery, department of hematology, department of tumor, department of neurology, department of endocrinology, department of general surgery, and department of gastroenterology. There was no statistical difference in the prevalence rate of nosocomial infections between the different scales of hospitals. The mean utilization rate of antibiotics was 35.24%, and the difference in the utilization rate of antibiotics between different scales of hospitals was significant (P〈0.05). All the surgi- cal departments ranked the top ten utilization rate of antibiotics except for the department of pediatrics and the general ICU. CONCLUSION The general ICUs of the traditional Chinese medicine hospitals are at high risk of the nosocomial infection; the larger the scale of the hospital is, the lower the utilization rate of antibiotics is. The tra- ditional Chinese medicine hospitals still need to intensify the control of the nosocomial infections.
