错配修复系统MLH1基因rs1800734多态性与肝细胞癌的关联研究 被引量:4

Study on the relationship between mismatch repair system MLH1 polymorphisms and hepatocellular carcinoma

摘要 目的研究错配修复系统MLH1基因rs1800734多态性与肝细胞癌遗传易感性的关系。方法采用以医院为基础的病例对照研究方法,运用荧光定量聚合酶链反应(polymerase chain reaction,PCR)和非条件Logistic回归分析MLH1基因型在两组中分布频率的差异,以及基因多态性和环境因素的交互作用。结果病例组MLH1基因位点AA、AG和GG各基因型频率分别为37.09%、48.57%和14.35%,对照组中基因型频率分别为32.89%、47.46%和19.65%,差异无统计学意义(P=0.085)。多因素Logistic回归分析表明,与AA基因型相比,AG或者GG基因型的个体罹患HCC的风险OR值分别为1.102和1.544。病例组等位基因(A)频率(61.37%)较对照组(56.62%)增高(2=4.22,P=0.040),A等位基因携带者患肝细胞癌的危险性是G等位基因携带者的1.217倍。交互作用分析结果表明MLH1基因多态性与肿瘤家族史、乙型肝炎病毒表面抗原(epatitis B surface antigen,HBsAg)阳性之间均存在交互作用(P<0.001),OR值分别为4.763和16.967。结论 MLH1基因可能是广西人群肝细胞癌的危险因素之一,其多态性与肿瘤家族史、HBsAg阳性之间在肝细胞癌发生中存在交互作用,能增加罹患肝细胞癌的风险。 Objective To study the relationship between mismatch repair system MLH1 polymorphisms and hepa- toeellular carcinoma. Methods A hospital-based case-control study on hepatocellular carcinoma was conducted. Real- time polymerase chain reaction and unconditional Logistic regression were used to detect the difference of frequencies in ease and control groups, then the interactions between polymorphisms and environmental factors were analyzed. Results The frequencies of MLH1 AA, AG and GG genotype in hepatocellular carcinoma group were 37.09%, 48.57% and 14. 35%, respectively, while 32. 89%, 47. 46% and 19. 65% in control group, respectively. The distribution was not sig- nificant different between two groups(P =0. 085). The odds ratio (OR) of MLH1 rs1800734 AG and GG genotype com- pared to AA were significantly increased, adjusted OR were 1. 102 and 1. 544, respectively. Individuals carrying MLH1 rs1800734 A aUelic gene had a significantly increased frequency compared to those with MLH1 rs1800734 G allelic gene ( adjusted OR = 1.217,95 % CI: 1. 009-1. 468 ). Interactions were found between infection of HBsAg and MLH 1 gene, his- tory of cancer and MLH1 gene ( P 〈 0. 001 ) , adjusted OR were 16. 967 and 4. 763, respectively. Conclusions MLH1 rs1800734 genotype might increase the risk of hepatocellular carcinoma. Interactions between HBsAg infection, history of cancer and MLH1 gene were observed in hepatoeellular carcinoma.
