摘要
目的 探讨高位子宫下段横切口在瘢痕子宫（CSP）产妇的剖宫产术中的临床效果.方法 选择2011年9月-2013年4月接诊的CSP产妇114例,其中57例产妇行高位子宫下段横切口手术（观察组）,另外57例行传统剖宫产术（对照组）.对两组胎儿娩出时间、手术时长、术中出血量、术后排气时间以及并发症情况进行统计,并对两组患者切口愈合及瘢痕形成情况进行观察.结果 观察组胎儿娩出时间、手术时长、术中出血量、术后排气时间分别为（12.1±2.7）min、（49.2±2.4）min、（332±108）ml和（21±6）h,对照组分别为（11.6±1.9）min、（52.1±1.4）min、（239±87）ml和（39±7）h,两组术中出血量、术后排气时间相比,差异有统计学意义（均P＜0.05）;观察组并发症发生率为5.3％（3/57例）,对照组为10.6％（6/57例）,差异无统计学意义（P＞0.05）;观察组愈合甲级51例（89.5％）,瘢痕明显11例（19.3％）,而对照组愈合甲级33例（57.9％）,瘢痕明显28例（49.1％）,差异有统计学意义（P＜0.05）.结论 高位子宫下段横切口在CSP产妇的剖宫产中术后恢复时间缩短,并发症较低,且切口愈合好,瘢痕不明显,适合对术后美观要求较高的产妇,值得在临床上进一步推广.
Objective To observe and explore the clinical effects of the lower uterine segment transverse incision cesarean section for caesarean scar pregnancy（CSP）.Methods 114 cases with CSP in hospital from September 2011 to April 2013 were selected.57 cases accepted lower uterine segment transverse incision cesarean section,as the observation group.57 cases accepted traditional cesarean section,as the control group.Fetus time,surgery time,blood loss,postoperative exhaust time and complications in two groups were recorded,and wound healing and scar formation were observed.Results Fetus time,surgery time,blood loss and exhaust time in observation group were （12.1±2.7） min,（49.2±2.4） min,（332±108） ml and （21±6） h,and （11.6±1.9） min,（52.1±1.4） min,（239±87） ml and （39±7） h in control group.The difference of blood loss,postoperative exhaust time in the two groups were statistically significance （P＜0.05）.The complication rate for observation group and the control group was 5.3％ （3/57） and 10.6％ （6/57） （P＞0.05）.grade A healed in 51 patients （89.5％）,scar evident in 11 cases （19.3％） in observation group,while grade A healed in 33 cases （57.9％）,scar evident in 28 cases （49.1％） in the control group.The difference was statistically significant （P＜0.05）.Conclusion The lower uterine segment transverse incision cesarean section for caesarean scar pregnancy can shorten the postoperative recovery time,and is with less complications and better wound healing,scar is not obvious,so is suitable for patients with high postoperative aesthetic requirements,thus is worthy of further promotion in clinical.
