脑脊液引流在脑出血手术的作用 被引量:4

摘要 目的比较基底核脑出血经小骨窗外侧裂与经颞叶皮质清除血肿的引流情况及疗效,并分析脑脊液引流的作用。方法选择基底核脑出血47例,联合引流组经外侧裂手术28例,血肿引流组经颞叶皮质造瘘手术19例。观察术后脑脊液及血肿引流量,术后3 d及2周意识情况,以及术后3个月的日常生活能力(ADL),并进行统计学分析。结果联合引流组与血肿引流组的平均引流量分别为270 ml与82 ml。术后3 d意识障碍者联合引流组24例,血肿引流组17例,两组间差异显著(P=0.041)。术后2周意识障碍联合引流组20例,血肿引流组14例,两组间差异无显著性。术后3个月两组间ADL分级,经统计学分析差异显著(P<0.05)。结论脑脊液充分引流可使病人较早恢复意识,对远期预后改善明显。
