摘要
目的:总结非小细胞肺癌EGFR突变状态与脑转移的研究进展,探讨EGFR突变非小细胞肺癌脑转移治疗策略。方法:应用PubMed和CNKI期刊全文检索系统,以"非小细胞肺癌脑转移、靶向治疗和全脑放疗"为关键词,共检索到2004-2013年相关文献270篇,其中英文文献160篇,中文文献110篇。纳入标准:1)非小细胞肺癌脑转移与EGFR突变状态关系;2)表皮生长因子受体酪氨酸激酶抑制剂(epidermal growth factor receptor-tyrosine kinase inhibitors,EGFR-TKIs)在非小细胞肺癌脑转移中的作用;3)非小细胞肺癌脑转移全脑放疗。根据纳入标准分析文献30篇。结果:EGFR突变NSCLC脑转移患者TKIs治疗敏感,同时中枢转移灶放疗亦敏感,而EGFR野生型及TKIs获得性耐药患者中枢转移灶放疗抵抗;EGFR突变状态与NSCLC脑转移患者TKIs治疗疗效、放疗疗效、生存及预后明显相关。结论:EGFR基因突变状态在指导NSCLC脑转移患者治疗、预测预后方面具有重要的作用。
OBJECTIVE:To summarize the progress of brain metastasis in non-small cell lung cancer with different EGFR gene status,discuss non-small cell lung cancer with EGFR mutations brain metastasis treatment strategies.METHODS:The full text database of PubMed and CNKI were searched,and the words"non-small cell lung cancer with brain metastasis","targeted therapy"and"whole-brain radiotherapy"were used as key words.Totally 270related articles were retrieved between the year of 2004and 2013with 160papers of literature in English and 110in Chinese.The selection standards for the date related:1)Non-small cell lung cancer with brain metastasis relationship with EGFR mutation status; 2)Epidermal growth factor receptor tyrosine kinase inhibitors in non-small cell lung cancer with brain metastasis;3)Nonsmall cell lung cancer with brain metastasis whole brain radiotherapy.According to the standards mentioned above,30papers were finally selected and analyzed.RESULTS:NSCLC patients with brain metastases and EGFR mutations are sensitive to TKIs,but the central nervous metastases also are sensitive to radiotherapy,whereas the brain metastases patients with wild-type and TKIs acquired resistance are radiation resistance.There are significantly correlation between EGFR mutations status and NSCLC patients with brain metastases to TKIs therapy,radiotherapy,survival and prognosis.CONCLUSION:EGFR mutation status are impotant in guiding treatment,predicting the prognosis of NSCLC patients with brain metastases.
