肺炎克雷伯菌β-内酰胺酶基因型分布 被引量:6

Detection of characterization of β-lactamases in Klebsiella pneumoniae isolates in Guangzhou

摘要 目的阐明广州地区肺炎克雷伯菌β-内酰胺酶基因型分布。方法用药敏纸片法对67株耐三代头孢菌素类抗生素的肺炎克雷伯菌进行超广谱β-内酰胺酶(ESBLs)表型筛选,采用PCR检测β-内酰胺酶耐药基因,对部分ESBLs菌株进行脉冲场凝胶电泳(PFGE)。结果 67株菌中有58株菌ESBLs表型阳性,β-内酰胺酶基因检测显示,44株产CTX-M、43株产SHV(其中10株产超广谱的SHV)、14株产OXA-1群、38株产TEM,7株菌产AmpC。产CTX-M的菌株有20株产CTX-M-1群(包括15株产CTX-M-15)和24株产CTX-M-9群(包括14株产CTXM-14)。PFGE结果显示产ESBLs菌株同源性低。结论广州地区耐三代头孢肺炎克雷伯菌以产CTX-M-1群和CTX-M-9群ESBLs为主。 Objective To investigate β-1actams distribution of Klebsiella pneumoniae in Guangzhou. Methods ESBLs phenotype was screened by disk diffusion method.PCR was used to identify β-lactamases.Pulsed field gel electrophoresis (PFGE) was performed to investigate the homology of ESBL-producers. Results Fifty-eight isolates of K.pneumoniae were confirmed for producing ESBLs,including 44 CTX-M, 43 SHV (in which there 10 produced ESBL),38 TEM and 14 OXA-1. In addition, 7 out of 58 isolates produced AmpC β-1actamases.Among CTX-M producers, 20 isolates produced CTX-M-lgroup (15 producing CTX-M-15) and 24 produced CTX-M-9group (14 producing CTX-M-14). PFGE indicated that the CTX-M-producing isolates were not homologous. Conclusion CTX-M-lgroup and CTX-M-9 group are the predominant ESBLs types in Guangzhou, China.
