摘要
目的研究腹腔镜下内支架胆肠吻合的安全性与可行性。方法 18只小型猪被随机分成两组:一组开腹行间断缝合胆管空肠吻合术(n=9,对照组),另一组行腹腔镜下内支架胆管空肠吻合术(n=9,实验组)。结果实验组手术时间为(60.2±3.7)min,对照组为(46.4±4.3)min,实验组手术时间显著长于对照组,差异具有统计学意义(P<0.05);术后6个月实验组胆肠吻合口内径为(3.8±0.3)mm,显著大于对照组的(2.9±0.4)mm,两组差异具有统计学意义(P<0.05);对照组小型猪发生吻合口狭窄者2例;所有小型猪内支架管都在58d内经肛排出。结论腹腔镜下内支架胆肠吻合安全易行。
Objective To explore the feasibility and safety of laparoscopic cholangiojejunostomy(CJ) with an intra-anastomotic biliary stent. Methods 18 minipigs with small-caliber bile ducts were randomly divided into two groups. One group performed open CJ with interrupted sutures without any stent(n=9,control group). One group performed laparoscopic CJ with an intra-anastomotic biliary stent(n=9,experimental group). Results The mean operation time of the experimental group was(60.2±3.7) minutes and that of the control group was(46.4±4.3) minutes, the operation time of experimental group was longer than control group,it had significant difference(P〈0.05). After 6 months, the CJ caliber of the experimental group was larger(3.8mm±0.3mm) than the control group(2.9mm±0.4mm),it had significant difference(P〈0.05). Two cases of the control group occurred anastomotic stricture. All the stents were discharged by anus in 58 days. Conclusion The laparoscopic CJ with an intra-anastomotic biliary sten is easy and safe to perform.
