摘要
目的:探讨老年女性脑梗死( CI)患者血清雌激素水平的变化与血脂水平、梗死面积之间的相关性。方法选取老年女性CI患者150例作为观察组,选取150例健康老年女性作为对照组,测定性激素、血脂,利用CT测定新发病灶的数量和各病灶的最大面积。结果观察组患者血清雌二醇(E2)(84.32±34.16)pmol/L、甘油三酯(TG)(2.11±0.85)mmol/L、低密度脂蛋白(LDL)(4.12±1.17)mmol/L,水平均高于对照组,观察组患者高密度脂蛋白(HDL)(0.87±0.43)mmol/L,水平低于对照组,差异均有统计学意义(t=2.43、3.88、6.91、9.98,均P<0.05)。同时在多灶型CI中,血清E2、TG和LDL水平较单灶型CI升高,差异有统计学意义(t=2.06、3.08、2.47,均P<0.05);血清胆固醇(Chol)、HDL水平较单灶型CI降低,梗死面积较单灶型CI减小,差异有统计学意义(t=2.37、3.01、13.43,均P<0.05)。结论老年女性CI患者性激素E2的升高不仅是一种应激反应,而且可能通过影响血脂代谢的机制,缩小梗死的面积。
Objective To investigate the elderly female patients with cerebral infarction ( CI) serum estrogen levels and changes in lipid levels ,and the correlation between infarct size .Methods 150 elderly female patients with cerebral infarction were chosen as the observation group;150 cases of healthy elderly women as the control group .The sex hormone levels and lipid levels were measured .The number of new lesions and the maximum area of the lesion were detected by 64-slice spiral CT.Results The serum estradiol(E2)was(84.32 ±34.16)pmol/L,triglyceride (TG)was(2.11 ±0.85)mmol/L,low density lipoprotein(LDL)was(4.12 ±1.17)mmol/L,all were higher in the ob-servation group.while the high-density lipoprotein(HDL)levels was(0.87 ±0.43)mmol/L,lower than that of the control group,the differences were statistically significant (t=2.43,3.88,6.91,9.98,all P〈0.05).While multifocal CI,serum E2 ,TG and LDL levels than a single lesion type CI increased ,the difference was statistically significant ( t=2.06,3.08,2.47,all P〈0.05);The serum Chol,HDL levels than a single lesion type CI decreased infarct size com-pared with single stove type CI decreases ,the difference was statistically significant ( t=2.37,3.01,13.43,all P〈0.05 ) .Conclusion Elderly patients with CI female sex hormone E 2 elevated not only a stress response ,but also may influence the mechanism of lipid metabolism ,reduce infarct size .
