摘要
目的探讨不同时机应用抗菌药物在普外科围术期中的临床效果,为其临床相关研究提供参考依据。方法回顾性收集2002年4月-2013年4月入住普外科的150例患者临床资料,分析其病史及相关信息,根据使用抗菌药物时机不同分为术前应用抗菌药物(A组)、术中应用抗菌药物(B组)、术后应用抗菌药物(C组),对3组患者的住院时间、切口感染等进行比较。结果平均住院时间、切口愈合时间C组患者为(15.3±2.6)d、(9.4±3.1)d,高于A组的(10.8±3.5)d、(6.6±2.8)d及B组的(6.9±2.6)d、(5.2±3.2)d,B组住院时间、切口愈合时间低于A组,差异均有统计学意义(P<0.05);切口感染率A、B、C组分别为8.0%、2.0%、20.0%,3组比较,差异有统计学意义(P<0.05)。结论手术时间>3h的患者,除在术前及术后使用抗菌药物外,在术中加用抗菌药物可以更有效的降低普外科围手术期感染发生风险;且住院时间及切口愈合时间亦比较短,因此,在该类手术术中加用抗菌药物有效,值得临床推广应用。
OBJECTIVE To study the clinical effect of using antibiotics in different times in the general surgery perioperative period, so as to provide reference basis for the clinical. METHODS From Apr. 2002 to Apr. 2013, a total of 150 patients admitted to the general, surgery department in our hospital, and a detailed analysis of its history and related information were taken retrospectively. According to the timing of the use of antibiotics the patients were divided into three groups A, B, C, group A patients using antibiotics preoperatively, B group of patients using antibiotics intraoperatively, group C with postoperative use of antibiotics. For three groups, hospitalization time, incision infection and so on were compared. RESULTS The average length of hospital stay and incision healing time were (15.3±2.6)d and (9.4±3.1)d of group C, which were higher than group A (10.8± 3.5) d and (6.6±.8)d and group B (6.9±2.6)d, (5.2±3.2)d, and group B were higher than group A about these two items, and the difference had statistical significance (P〈0.05). The rate of incision infection in three groups were 8.0% in group A, 2.0% in group B and 20.0% in group C, and the comparison in three groups were significantly different (P〈0.05). CONCLUSIONS Except in the preoperative and postoperative use of antibiotics, the patients with more than 3 hoursr operation time could more effectively reduce the risk of general surgery perioperative infections by using antibiotics during operation, which can also reduce the time of hospitalization and incision healing, so it's worthy to popularize by adding antibiotics effectively in such surgery.
