摘要
目的 总结尸肾移植 83例的临床经验 ,分析常见并发症的原因及防治措施。方法 回顾分析 83例尸肾移植一般情况 ,手术方法、免疫抑制剂的应用 ,并发症的发生情况等。结果 6 1例肾功能恢复顺利 ,余 32例中 3例发生超急性排斥反应 ,9例发生急性排斥反应 ,10例发生急性肾小管坏死 ,3例发生尿瘘 ,2例输尿管梗阻 ,1例肺部感染 ,1例骨髓抑制 ,2例消化道出血 ,死亡 1例。主要死亡原因肺部感染。结论 术后影响移植肾成功的主要原因是急性排斥反应及急性肾小管坏死。感染是影响受者存活的主要原因。合理应用免疫抑制剂应高度重视。
Objective To discuss the cause,treatment and prevention of the complications of kidney transplantation based on the clinical experience in 83 cases of kidney transplantation.Methods A retrospective analysis was made on the chinical data,operative technigues,application of immunosuppression,and complications of the 83 cases.Results After the operations,there were 3 cases of hyperacute rejection,9 acute rejection,10 acute tubular necrosis(ATN),3 urinary fistula 2 ureteral obstruction,1 case of pulmonary infection,1 case of bone marrow suppression,and 2 cases of digestive tract bleeding.One patient died of pulmonary infection.Conclusion The major causes of dysfunction of the transplanted kidney in the postoperative sfage are acute rejection and ATN.Infection is mainly related to the mortality of recipients.Emphasis should be laid on the appropriate administration of immunosuppressive agents.
