摘要
目的探讨能谱CT在婴幼儿肝移植受体术前影像评估中的应用价值。方法对30例肝移植受体患儿做术前能谱CT检查。对肝动脉、门静脉和肝静脉做单能CT和混合能量CT的CT血管造影(CTA)。测量患儿的肝脏体积并与术中切除标本对照。结果单能CT在50 keV时的肝内血管CTA成像质量优于混合能量CT(P<0.01)。CT测量肝体积与术中测得数据有良好对应(P>0.05)。患儿的血管变异情况均在CTA得以显示。结论能谱CT单能CTA和肝体积测量作为婴幼儿肝移植受体术前评估的一种方法,可以获得良好的对比噪声比,保证血管CT成像的质量和评估的准确性。
Objective The purpose of our study was to discuss the utilization of spectral CT in preoperative imaging evaluation of pediatric liver transplantation recipients. Methods Thirty pediatric patients underwent the spectral CT before operations. To examine the hepatic artery, portal and hepatic vein, we conducted CTA reconstruction of monochromatic and combined energy CT, and then compared the liver volume from CTA reconstruction with the surgical weighting. Results On CTA liver reconstruction, 50 keV monoenergetic CT displayed better image quality than the combined energy reconstruction ( P 〈0. 01 ). The difference of liver volume between CT reconstruction and surgical weighting are correspondent ( P 〉 0.05 ). All the anatomical variations of recipients were confirmed by the operation result. Conclusion As a method for evaluation of pediatric liver transplantation recipients, to reconstruct the monochromatic CTA imaging and measure the volume of liver by using spectral CT is a way to evaluate liver transplantation recipients. It can obtain the best contrast noise ratio, guarantee the quality of vessel reconstruction image and the accuracy of assessment.
