摘要
目的探讨左氧氟沙星联合头孢他啶治疗心力衰竭患者肺部感染的疗效及对白细胞介素(IL-6)、肿瘤坏死因子(TNF-α)和B型脑钠钛(BNP)水平的影响。方法选择2012年5月-2013年5月接受治疗的心力衰竭并发肺部感染患者42例,给予左氧氟沙星联合头孢他啶治疗,比较患者治疗前后的临床症状及IL-6、TNF-α、BNP水平变化,采用SPSS17.0统计软件进行数据处理。结果 42例心力衰竭并发肺部感染患者,治疗显效13例、有效24例、无效5例,总有效37例,总有效率为88.10%,症状积分治疗前(107±11)分、治疗后(31±7)分,差异有统计学意义(P<0.05);治疗前患者IL-6、TNF-α、BNP水平分别为(1.8±0.4)ng/L、(1.7±0.7)ng/L、(2.0±0.9)pg/ml,治疗后分别为(0.4±0.9)ng/L、(0.5±0.4)ng/L、(0.5±0.6)pg/ml,治疗前后比较差异有统计学意义(P<0.05);治疗过程中无1例患者出现药物不良反应。结论左氧氟沙星联合头孢他啶治疗,能够提高心力衰竭并发肺部感染患者的临床疗效,且改善IL-6、TNF-α、BNP水平,值得临床推广。
OBJECTIVE To investigate the affect of levofloxacin combining ceftazidime treatment of left ventricular failure with pulmonary infection and its impact on IL-6 ,TNF-αand BNP levels .METHODS A total of 42 patients with left heart failure complicated with pulmonary infection from May 2012 to May 2013 in our hospital were collected and given levofloxacin combined with ceftazidime treatment .The changes before and after treatment of clinical symptoms and IL-6 ,TNF-α,BNP levels in patients were compared .SPSS17 .0 software was used for statistical analysis .RESULTS Among 42 patients with left heart failure complicated with pulmonary infection ,13 cases with obvious effect ,24 with effect ,5 with no effect and the total cases of effective were 37 cases ,accounting for 88 .10% .Before treatment ,symptom scores were (107 ± 11) points ,after treatment (31 ± 7) points .There was significant difference (P〈0 .05) .The IL-6 ,TNF-α,BNP levels before treatment were (1 .8 ± 0 .4) ng/L , (1 .7 ± 0 .7)ng/L and (2 .0 ± 0 .9) pg/ml ,after treatment were (0 .4 ± 0 .9) ng/L ,(0 .5 ± 0 .4) ng/L and (0 .5 ± 0 .6)pg/ml ,and there were significant differences (P〈0 .05) .There was no untoward effect to drugs during the treatment .CONCLUSION Levofloxacin combined with ceftazidime can improve left ventricular failure with pulmonary infection clinical efficacy and improve IL-6 ,TNF-α,BNP level ,which is worth to be promoted in clinical .
