胎膜早破引起新生儿感染的病原菌分析 被引量:24

摘要 目的:探讨分析胎膜早破引起新生儿感染的病原菌分布,以降低新生儿感染的发生。方法回顾性分析2008年1月-2012年12月1584例胎膜早破病例的临床资料,其中326例新生儿发生感染,对其进行病原菌检测,采用SPSS 17.0软件对数据进行统计分析。结果326例新生儿中有217例为感染性肺炎,占66.57%,68例为败血症,占20.86%;胎膜早破至出生时间≤12 h者23例、13~72 h 249例、>72 h 54例,分别占7.06%、76.38%、16.56%;294例新生儿出现感染病灶局部分泌物和血培养阳性,阳性率为90.18%;共培养出病原菌312株,其中革兰阳性菌194株、革兰阴性菌113株、真菌5株,分别占62.18%、36.22%、1.60%;不同的胎膜早破时间与病原菌阳性率之间,差异有统计学意义( P<0.05);237例早产儿病原菌检出率为97.04%,89例足月儿病原菌检出率为71.91%,两者比较,差异有统计学意义( P<0.05)。结论胎膜早破与新生儿病原菌感染关系密切,临床应积极做好胎膜早破与新生儿感染的预防与治疗工作。 OBJECTIVE To investigate and analyze the distribution of neonatal infectious pathogens caused by premature rupture of membranes ,so as to reduce the occurrence of neonatal infections .METHODS A retrospective analysis was condueted on the clinical data of 1 ,584 cases of premature rupture of membranes hospitalized from Jan 2008 to Dec 2012 ,and then the pathogens detected were that caused 326 cases of neonatal infections ,and the data were analyzed after detection with SPSS 17 .0 software .RESULTS Among 326 cases of neonatal infections ,there were 217 cases suffered from infectious pneumonia ,accounting for 66 .56% of total cases ;68 cases of sepsis , accounting for 20 .86% .There were 23 cases'time interval between premature rupture of membranes to birth less than 12 hours ,and that of 249 cases was 13 hours to 72 hours ,that of 54 cases was more than 72 hours , accounting for 7 .06% ,76 .38% and 16 .56% respectively .The detection for secreta and blood culture taken from infectious focus of 294 cases were positive , and the positive rate was 90 .18% . There were 312 strains of pathogens ,194 strains of gram-positive bacteria ,113 strains of gram-negative bacteria and 5 strains of fungi , accounting for 62 .18% , 36 .22% and 1 .60% . The difference between the time of premature rupture of membranes and the positive rate of pathogen had statistical significance ( P〈 0 .05 ) . The detection rate of pathogens in 237 premature infants was 97 .04% ,and that in 89 full-term infants was 71 .91% ,so the difference between them had a statistical significance (P〈 0 .05) .CONCLUSIONS Premature rupture of membranes has a close connection with neonatal pathogen infections ,and it is necessary to conduct well prevention and treatment for premature rupture of membranes and neonatal infections .
