摘要
目的：研究探讨施行角膜层间分离清刮术（ Corneal interlameller clean curettage ，CICC）治疗大泡性角膜病变（ Bullous keratopathy ，BK）的临床效果。方法：选取我院从2011年4月～2012年5月被确认为BK病患42例，均有角膜上皮不同程度水肿，且失去光泽，有1个或多个大水泡隆起，直径范围约为1～3 mm，部分水泡破裂后，用荧光素染色呈阳性。对其予以角膜层间分离清刮术治疗，观察其疗效以及术后并发症情况，对手术前后病患视力情况记录并进行对比。结果：观察术后疗效及并发症情况，9例病患于手术后2天开始疼痛减轻，15例病患在手术后1周各症状均明显减轻，所有病患于2周内眼部疼痛、畏光以及流泪等症状均解除。病患角膜面平整、光滑，结膜充血情况得以减轻，且角膜逐渐恢复透明。2年之内坚持随访，均无复发及其他明显并发症发生。病患手术前后视力情况对比，手术后病患视力为手动及眼前指数的病患所占比例分别为0％、14．29％，显著低于手术前的47．62％、35．71％。且视力为0．01～0．02及0．03～0．04的病患所占比例分别为59．52％、26．19％，显著高于手术前的16．67％、0％。差异均有统计学意义（均P＜0．05）。结论：CICC治疗BK，疗效显著，可有效清除角膜大泡病变，解除病患疼痛，并能够恢复病患视力，无并发症发生，安全性高，具有积极意义，值得推荐。
Objective：To study clinical effect of discussion of corneal interlameller clean curettage （ CICC） in the treatment of bullous keratopathy （ BK） .Methods：Our hospital chose from April 2012 to May 2013 of BK patients , 42 cases had varying degrees of corneal epi-thelial edema, and loss of luster, there were one or more large blisters uplift, diameter range of about after 1 ~3mm, part broken blis-ters, positive staining with fluorescein .It＆#39;s been clear separation between the layers of the cornea curettage treatment to evaluate efficacy and postoperative complications , visual acuity before and after the surgery the patient was recorded and compared .Results：Observed post-operative effects and complications , 9 cases of patients at the beginning of pain relief 2d after surgery,15 cases of patients one week after surgery＆#39;s all symptoms were significantly reduced in all patients within 2 weeks of eye pain ,fear light and tearing and other symptoms were released .Patients with corneal surface smooth, conjunctival hyperemia alleviated the situation , and the gradual recovery of corneal trans-parency .Adhere to follow-up within two years , no recurrence and other complications occurred .Visual contrast to the situation before and after surgery for patients , with visual acuity of patients after surgery＆#39;s proportion of manual and front indices were 0%, 14.29%, 47. 62%, which was significantly lower than before surgery of 35.71%.And visual acuity was 0.01 to 0.02 and 0.03 to 0.04＆#39;s proportion of patients were 59 .52%, 26 .19%, significantly higher than 16 .67%before surgery ,0%.Differences were statistically significant （ P ＆lt;0.05）.Conclusion：CICC in the treatment of BK, has a significant effect, can effectively remove corneal lesions cannon , relieve pain pa-tients, and be able to restore vision in patients with no complications , which is safe,has a positive meaning.It is recommended .
